项目摘要

顎関節同時多層断層エックス線フィルムをデジタル画像処理して変形性顎関節症患者の関節腔隙の狭小化等に絞り検討を加えた. その際, 最終的に下顎頭, 関節窩の位置関係をグラフ化する事が出来るが, 関節結節最下点(E)と側頭鱗鼓室裂溝の位置(F)を指示して, この両点を結ぶ上方の関節腔隙の面積を画素数で表わし, また(E)と(F)の間の距離も求められるようにした. 上記の画像処理時間は約10分程度で比較的短時間の内に作業が完了できる. 又この画像処理のポイントは, 本来辺縁がぼけている下顎頭, 関節窩を明瞭に抽出する作業にあることは言うまでもない.さて当教室は過去3年間顎関節症患者150余名に対し, 顎関節同時多層断層エックス線撮影を施行したが, その内, 健常者のグループ, 臨床的には顎関節症を示しているが, 断層エックス線フィルム上では異常所見を示さない26名のグループ(52顎関節), 更に変形性顎関節症を示す24名(34顎関節)の厳選したグループとに大別し, 夫々の関節腔隙の狭小化, 関節結節最下点と側頭鱗鼓室裂溝を結ぶ距離, 下顎頭-関節窩間距離などを計測した.<結果>1.変形性顎関節症を示した症例の内, Osteophyteを示す症例において関節腔隙の極端な狭小化を認めた. 2.断層エックス線フィルムで, 異常所見を認めないグループと, 健常者グループとは, 関節腔隙, 及びE-F間距離とも有意差を認めなかった. 3.変形性顎関節症のグループは, 変形を示さないグループに比べて, E-F間距離が短く, 関節腔隙も狭小化していた.
