前年度と同手法によって慢性高血圧犬を作成してきたが、前年度と同様に今年度も高血圧犬を4週間以上生かして置くことが必ずしも容易ではなかった。最も多い合併症は胸水が貯留して術後1週間以内に死亡することであった。感染症が主な原因と考えられるが、術中の清潔操作及び術後管理に関しては十分注意を払っているので、感染を起こすか否かは術前の成犬の健康状態に大きく依存している様に思える(米国の様に管理が万全な養犬所ら入ってくる成犬とは異なり、我々の購入する犬は必ずしも健固ではないのが日本の現状である)。また、血圧が短期間(腎rapping後2-4日目)に急上昇して食欲が無くなり、下血や嘔吐等を繰り返して観察不能になってしまう例も多い。現在、上述した様な脱落犬を少しでも無くするように努力しつつ(例えば抗生物質の増量や健康犬の厳選等)症例を蓄積中である。しかし、この様な中でも今回も興味ある新知見を得ている。2頭の犬では術後3週間で最高血圧(PSP)が170-200mmHgまで上昇した。心尖部(Ax)、心基部(Bx)の壁厚は何れも減少傾向(6-8%)を示したが(左室の拡大)、Bxの壁厚増増加率(%W)は不変であったのに対しAxの%Wは著明に低下していた。この所見は、後負荷上昇の初期にはAxにafterload mismatchが生じている事を示唆している。この時期のtime constant(T)は著明に変化しなかったが、陰性(-)dp/dt上行脚のpatternは下に凸(等容弛緩期のregional asynchronyを示唆する:著者ら;Am J Physiol,1979)であった。即ち、高血圧の初期にはAxとBx間に収縮期asynchronous wall motionが生じ、弛緩期にはrelaxation異常が起こる事を示唆する所見である。これらの新知見が症例数を増し、かつ長期高血圧になるとどうなるのかを更に検討中である。

The new knowledge is obtained from the new knowledge. The maximum blood pressure (PSP) increased from 170 to 200mmHg in 2 dogs 3 weeks after operation. The wall thickness of apex (Ax) and base of heart (Bx) decreased by 6-8% while the wall thickness of left ventricle increased by %W. This observation shows that the initial period of load rise is Ax and afterload mismatch. The time constant(T) of this period is opposite to that of the light, negative (-)dp/dt ascending foot pattern is opposite to that of the downward convex (isovolemic relaxation period regional asynchronous). That is, during the initial period of high blood pressure, asynchronous wall motion occurs, relaxation occurs, and abnormal events occur. The number of cases increased, and the number of cases of chronic hypertension increased.
