1984年4月より1988年3月までの4年間に105例の泌尿性器癌(腎癌19例、腎盂癌3例、尿管癌4例、浸潤性膀胱癌35例、膀胱上皮内癌13例、前立腺癌10例、泌尿性器癌よりの転移巣15例、その他6例)に対し、局所温熱療法、および放射線もしくは化学療法との併用を試みた。105例中CR9例、PR25例、MR23例が得られた。癌の局所浸潤による疼痛を有した28例のうち23例に著明な除痛効果が得られた。病理組織学的には、温熱療法により、画像診断上MR以上の腫瘍縮小が得られた膀胱癌10症例について、大量の基準に準じて検討を行った。10例中5例でiiB以上の変化がみられ、著明な細胞変性や壊死が認められた。これらの変化は腫瘍辺縁部よりも中心部に著明な傾向がみられた。また血管系については、ほぼ全例にcongestion,edemaおよび一部でhyalinous stroma出現などの変化が認められた。免疫学的検討では局所温熱療法の免疫能を検索する目的で、105例中33例で、治療前後のリンパ球サブセットをモノクロナール抗体を用いて測定した。その結果、今回検討した泌尿性器癌患者においては、良性疾患群と比較してリンパ球サブセットの比率に差は認められなかった。また、局所温熱療法単独では、末梢血リンパ球に及ぼす影響は極めて軽微で、この点から局所温熱療法は免疫能の低下した悪性腫瘍患者にとって有用な治療法であると考えられた。

During the 4 years from April 1984 to March 1988, 105 cases of urogenital carcinoma (renal carcinoma 19 cases, renal pelvis carcinoma 3 cases, urinary tract carcinoma 4 cases, invasive bladder carcinoma 35 cases, bladder intraepithelial carcinoma 13 cases, anterior adenocarcinoma 10 cases, urogenital carcinoma 15 cases, and other 6 cases) were examined. CR9 cases, PR25 cases, MR23 cases. The cancer invasion caused pain in 28 cases and pain in 23 cases. Pathological histology, thermotherapy, imaging diagnosis, tumor reduction, bladder cancer, 10 cases, a large number of criteria, evaluation 5 out of 10 cases were diagnosed with iiB or above. Immunological evaluation of the immune function of local thermotherapy was carried out in 33 of 105 cases. The results of this study were compared with those of benign diseases in patients with urogenital cancer. The effect of local thermotherapy on peripheral blood circulation is very small and the effect of local thermotherapy on immune function is very useful for patients with malignant tumors.
