The whole of this study was to find out the demands on daily life for the diabetic patients with severe complications and to clarify the nurses' role in social support system in community.In the first part, we selected 46 diabetic patients with eye sight disturbance or renal failure who required others support in daily life from the diabetic out-patient clinic in Jichi Medical School Hospital. The survey was done by a questionnaire method designed to evaluate their health state, life style, relationship to family, doctor and other medical personnel, compliance and attitude to the regimen including the diet, drug or insulin injection. The history of diabetes education being provided in the past and their effect were also surveyed.In the second part, the situation of diabetes education program being provided by nurse was investigated in 108 hospitals in Tochigi prefecture, where the subjects of the study of first part were living. Additionaly, we visited the four institutions well known for conducting the excellent diabetes education programs in Tokyo area and Melbourne in Australia.The followings were concluded from this study :1) The nurses' role in the diabetes education program should be clarified and defined more clearly and the knowledge and skill of education by nurse should be strengthened more intensively.2) The education program should be organized in the routine prating.3) Proper social support by nurse will improve the quality of life of the diabetic patient with severe physical complication.

本研究旨在了解糖尿病严重并发症患者的日常生活需求，明确护士在社区社会支持系统中的作用。第一部分，从济池医学院医院糖尿病门诊选取46例有视力障碍或肾功能衰竭、日常生活需要他人支持的糖尿病患者。调查是通过问卷调查的方法进行的，目的是评估他们的健康状况、生活方式、与家人、医生和其他医务人员的关系、对包括饮食、药物或胰岛素注射在内的养生方案的依从性和态度。第二部分对前一部分研究对象居住的赤木县108所医院开展护士糖尿病教育项目的情况进行了调查。此外，我们还走访了东京地区和澳大利亚墨尔本四所著名的糖尿病教育机构，得出以下结论：1)明确护士在糖尿病教育计划中的角色，加强护士教育的知识和技能；2)将教育计划组织在日常工作中；3)护士给予适当的社会支持，提高糖尿病严重身体并发症患者的生活质量。
