1.末梢血中Fcεレセプタ-陽性細胞数の測定と同定:アトピ-性皮膚炎(AD)、アトピ-素因のない湿疹患者(ED)、健常人の末梢血単核球をFicoll-Hypagueにより単離する。FITC標識H107(モノクロ-ナル抗Fcεレセプタ-抗体)およびPhycoerythrin(PE)標識モノクロ-ナル抗T、B、単球血清(Leuシリ-ズ)を用いて、二重染色しFACScanで陽性細胞のsubset同定と数の測定を行った。重症、中等症ADでは、Fcεレセプタ-(R)陽性細胞は6%前後で、軽症AD、ED、健常人の約2%に比べて有意に増加していた。いづれのグル-プでも陽性細胞はB細胞であったが、重症、中等症ADに限って陽性細胞の約10%がT細胞および単球であった。また、FcεR陽性T細胞の表面形質を検討したところ、CD8陽性細胞の方が、CD4陽性細胞より有意に多かった。これらの結果より、AD皮疹の程度と末梢FcεR陽性細胞の割合は正に相関していると言える。2.皮疹におけるFcε陽性細胞の動態:ADおよびED患者から得た生検皮疹におけるFcε陽性細胞をH107とモノクロ-ナル抗T、B、単球血清を用いた蛍光抗体二重染色法により、クリオスタットで薄切した切片中で同定し、陽性細胞の浸潤を調べた。AD皮疹のみにFcεR陽性細胞がみられ、急性増悪部では浸潤細胞の約4%、また、慢性苔癬化部では、その%が陽性細胞であった。FcεR陽性細胞の約半数はT細胞で、B細胞はみられなかった。これらのT細胞はCD8優位であった。以上1、2の結果より、AD皮疹形成に、FcεR陽性細胞が何らかの形で関与しており、それが末梢血中の割合として反映される可能性が示唆された。CD8を表出するFcεR陽性T細胞は、皮疹の増悪に関わっていると思われた。

1. The number of Fcε-positive cells in peripheral blood was measured in patients with AD, ED, and healthy individuals. FITC labeled H107(anti-Fc ε-antibody) and Phycoerythrin(PE) labeled H107 (anti-T, B, and T lymphocytes) were used in the determination of subset identity of FACScan positive cells by double staining. In severe and moderate AD, Fcε-(R) positive cells decreased by about 6%, while in AD, ED and healthy individuals, the positive cells increased by about 2% compared with those in normal individuals. About 10% of the positive cells in severe and moderate AD were T cells. The surface morphology of Fcε R-positive T cells, CD8-positive cells and CD4-positive cells were studied intentionally. The results showed that the degree of AD rash was positively correlated with the cleavage of FcεR positive cells. 2. Dynamics of Fcε-positive cells in rash:AD and ED patients developed rash, Fcε-positive cells were detected in H107 and anti-T, B and B lymphocytes were detected in serum by double staining with photoantibodies. The FcεR positive cells in AD rash were about 4% in acute and 4% in chronic lichen. About half of FcεR positive cells were T cells and B cells. CD8 is the preferred position of T cells. The results of 1 and 2 above indicate the possibility of AD rash formation, FcεR positive cells formation and fusion in peripheral blood. CD8-positive Fcε R-positive T cells are associated with the development of rashes.
