全結腸以上小腸中央部までにおよぶ広範無神経節症の外科治療に関する実験的研究

先ず、研究課題について、S-Dラットの無神経節(0.1% benzalconium chrolideにて作成、長さ3cm)結腸(右、左)または回腸末端パッチ付加あり、或いは付加なし空腸引き下ろしモデルを作成した。パッチなし引き下ろしモデルでは全結腸、盲腸、小腸50%が切除され、手術が完全であっても平均生存日数は9.2±1.48日(n=5)で最長生存日数は11日であり、体重も術前の約半分に減少、衰弱が高度であった。パッチ付加引き下ろしモデルはプルスル-前の側々吻合のみで死亡する例が多く、また実際に根治術に至っても腸管癒着が高度で、剥離後の虚血や穿孔でほとんどの例が術後2〜3日で死亡し、どの部位のパッチが有効か評価できるには至らなかった。即ち、計画したモデルでは手術侵襲や術後の代謝変動があまりに大きく、経腸栄養剤投与の管理でもってしても困難であることが判明した。人間同様、長期的輸液管理が必要なのであろう(ラットに於いての実施は極めて困難であるが)。今後は術後管理を含めた技術的問題のクリアが必要となろう。次に、右結腸パッチの長さを1cmに短縮して3cmモデルと比較検討した。尚、プルスル-は技術的に可能な回腸下端とした。術後4週目での体重変化率は1cmパッチ群で8.8±8.70%(n=5)、3cmパッチ群で6.2±6.76%(n=5)であり、無パッチ群の-16.2±6.01%(n=5)に比べ有意に増加していた(p<0.001)。血液学的デ-タは各群ともほぼ正常値を示し、特に血清電解質には有意差はなく、代償能が働いていた。即ち、回腸末端を含む通常の全結腸型無神経節症に於いても無神経節結腸パッチ付加は有効で、しかも、そのパッチは短くてもよいことが判明した。パッチの短域化は、手術時間の短縮、患者への侵襲低減、また便塊停滞によるenterocolitisの発生抑制につながる可能性もあり、この方面からの更なる検討も必要であろう。
