项目摘要

慢性活動性EBウイルス感染症は、EBウイルスに対する特異的免疫不全症と考えられているが、本邦における実態は明らかではなく、その病因、病態に不明な点が多い。まず本症の実態を把握する目的でアンケ-ト調査を行い、回答の得られた37例につき検討した。その結果18例が数年以内に死亡するという、極めて予後不良の疾患であることが判明した。代表的死因としては肝、心不全(10例),悪性リンパ腫(2例),日和見感染症(2例),その他(4例)であった。一方欧米で報告されているXーlinked lymphoproliferative syndrome(XLP)とは異なり、本邦例では明らかな遺伝的背景はなく、またATLのような地域集積性はみられなかった。興味深いことは、5例に顆粒リンパ球増多症(LDGL)がみられ、検策したり症例は全てEBウイルスゲノムを有するNK細胞増多症であった。本症に対しては確立された治療法がなく、ILー2,IFN,ステロイド剤、jーgl製剤,acycloviaなどが用いられてきた。我々はILー2を中心とした治療で部分寛解にはなったが、hyersplenismが持続する2症例でsplenectomyを施行し、肝機能をはじめとして全身状態の著しい改善をみた。摘出されたspleenのcell suspensionを調べると、その70〜80%がNK細胞であり、しかもこれらにEBウイルスゲノムが認められた。本症の特徴的所見のひとつであるsplenomegalyが、spleenでのNK細胞の増殖に起因するのか否かに関しては、今後の多数例での検討結果を待たねばならないが、本疾患でのNK細胞増殖の場がspleenであり、それが無秩序に行われている可能性を示唆するものであろう。このようなsplenectomyをはじめとした包括的な治療大系の確立と病因,病態のさらなる解明が今後の課題である。

The treatment of this disease has been established by using IL-2,IFN, acyclovir, etc. The treatment of IL-2 is partial, hypersplenism is persistent, splenectomy is performed, and liver function is improved. The cell suspension of Spleen is adjusted to 70 ~ 80% of NK cells. The findings of the characteristics of this disease indicate the cause of splenomegaly and pleen NK cell proliferation. The results of future studies in most cases are pending. The field of pleen NK cell proliferation in this disease is likely to be disorderly. This includes the establishment of a large number of etiological factors, pathological changes and future problems.
