1.症例収集:腎Angiomyolipoma(AML)17例の手術標本を収集し,さらに肝AML2例も収集して対象とした。結節性硬化症合併例は3例である。多発例は2例で,いずれも結節性硬化症合併例である。2.光顕的検索:ジアスタ-ゼ消化PAS染色を全例で行ったところ、腎AML17例中11例で明らかな針状結晶様構造が認められた。切片全体に均等に認められるわけではなく、部分的に認められる。島状に集簇して出現してくることが多く、微細な顆粒(これもジアスタ-ゼ耐性PAS陽性)を有する細胞群に混じて認められることが多い(尚,この微細顆粒の出現は17例全例に認められる)。平滑筋細胞が血管周波様配列を示したり、上皮様大型細胞化した部分に認められることが多い。結晶構造が比較的多数認められたのは3例で、内2例は結節性硬化症合併例である。結晶様構造を認めた例で高血圧を呈した例は2例であるが、いずれも腎摘出後高血圧は改善していない。肝AMLでは微細顆粒を認める。3.電顕構的検索:1例で、上皮様大型細胞内に多数の顆粒とともに、明瞭な結晶構造を認めた。形態学的には胞巣状軟部肉腫の結造と極めて類似している。他の1例でホルマリン固定試料からの戻し電顕,肝AML1例でパラフィンブロックからの戻し電顕を行ったが、結晶構造は見出し得ず,多数の顆粒を認めるにとどまった。4.コンピュ-タ-解析:結晶構造の電顕写真を用いて画像解折を行うと、周期性など胞巣状軟部肉腫の結晶とほぼ一効している。5.免疫組織化学的検索:全例レニン抗体を用いた染色で陰性であり、結晶構造はレニンではないということは証明し得た。各種細胞細骨蛋白も結晶構造含有細胞には、ほとんど染色されず、この細胞の分化の特異性については確定し得なかったが、この染色にあたっては、特殊な前処置の必要性があるのかもしれず、今後の課題となると考えられる。
