速中性子線治療中の皮膚の変化をビデオ画像、画像ソフトを用いて解析した。平成5年10月-12月に速中性子線治療を行った30例、49部位を対象とした。速中性子線治療は週3回照射で、一回線量は平均102cGy(90-150)であった。皮膚線量はMODULEXより求め、画像集録時の皮膚線量は91.8-2903cGyであった。皮膚のビデオ撮影は同室同部位、同じ照明状態(照明スタンド使用)で週1回行った。総測定回数は226回であり、測定した照射部位(測定部位)は顔面65、頸部27、体幹前面73、体幹後面61であった。撮影したビデオ画像データを今回開発した画像ソフトに取込み、照射部位、非照射部位のRGB値を測定した。また肉眼的紅斑の程度(なし:0、軽度:1、中等度:2、高度:3)を同じビデオ画像より評価した。非照射部位のRGB値はR値185±50(2SD)、G値147±48(2SD)、B値89±44(2SD)であり、各測定部位で差がみられなかった。同一患者非照射皮膚の測定のばらつき(2SD)はR値20%(4.4-20)、G値24%(6.6-61)、B値33%(15-63)であった。各画像データの非照射皮膚のRGB値を100%とし、照射皮膚のRGP値の変化を解析した。各測定部位の肉眼的紅斑の程度とR値、G値、B値の相関係数は-0.02から-0.26であった。一方皮膚線量とRGB値の相関係数は-0.20から-0.59であり、肉眼的所見よりも強い相関を示し、回帰直線:RGB値=A+B≠皮膚線量(Gy)の傾きBは-4.6から-15.4であった。特に頸部のR値、G値、B値と皮膚線量の相関係数、傾きBは他測定部位よりも相関が高い傾向が見られ(表1)、ROI設定の再現性が良いためと思われた。各測定部位において皮膚線量とG値がR値、B値よりも相関が高い傾向が見られた。(表1)。RGB値は照射線量と皮膚の色調変化を数量的に表現する指標として有用であると考えられた。

For non-irradiated skin of the same patient, R value was 20%(4.4-20), G value was 24%(6.6-61), B value was 33%(15-63). The RGB values of the non-irradiated skin of each image are 100% and the RGP values of the irradiated skin are analyzed. The correlation coefficient of the degree of red spots with naked eye at each measured position is-0.02-0.26 The correlation coefficient between RGB value and skin line value is-0.20-0.59, the visual value is strong, and the regression line is:RGB value =A+B≠ skin line value (Gy) B-4.6-15.4. In particular, the correlation coefficient of R value, G value and B value of neck skin line, inclination B value and other measured parts tend to be high (Table 1), and the reproducibility of ROI setting is good. For each measured site, the skin line G value is R value, B value is B value, and the correlation is high. (Table 1). RGB is the indicator of the amount of radiation and the amount of skin tone change.
