多臓器不全(Multiple organ failure;MOF)の成因、憎悪因子として各種humoral mediatorが関与すると推測されている。この際の持続血液漉過(Continuous hemofiltration;CHF)は、これらhumoral mediatorの除去による病態改善の可能性と、一方では、漉過膜表面での補体の活性化に基ずくhumoral mediator生成による病態悪化の可能性も併せもつ。そこでMOF患者6例にCHF施行の際の血液、漉過液の各種humoral mediatorを経時的に測定して、それらと病体の関連を検討し、以下の成績を得た。1.CHF開始時及び24時間後の漉過膜前後の血液と漉過液のinterleukin(IL)-1alpha,betaおよびtumor necrosis factor(TNF)-alphaを測定したが、1例のCHF開始時の漉過膜後の血液のTNF-alphaが6pg/mlであったが他は何れも測定感度以下(<5pg/ml)であった。2.6-keto-prostaglandin(PG)F_<1alpha>,thromboxane(TX)B_2,11-dehydro-TXB_2の漉過率は各々28-100%,17-100%,12-100%と個体間及び採取時期での差が大で、またCHF開始時と24時間後の値に差を認めなかった。3.TXB_2から11-dehydro-TXB_2への変換率は38-95%であり、平均69%であった。4.6-keto-PGF_<1alpha>,TXB_2,11-dehydro-TXB_2のCHF24時間後の値は前に比して何れも増減半ばし、一定の傾向は得られなかった。5.TXB_2+11-dehydro-TXB_2/6-keto-PGF_1はCHF開始時11.8であったが24時間後は半数が増加、半数が減少し一定の傾向は得られなかった。以上の事から、IL-1,TNFは現時点での測定系での評価は不可能で、また6-keto-PGF_<1alpha>,TXB_2については測定可能なものの、CHFによる影響は一定の傾向が得られず、明確な結論は得られなかった。但し、TXB_2から11-dehydro-TXB_2への変換率は平均69%である事からTXB_2の評価には11-dehydro-TXB_2も併せて評価すべきで有ることが確認された。

Multiple organ failure (MOF) is a common cause of multiple organ failure. Continuous hemofiltration (CHF) is the most common form of chronic inflammation, and the most common form of chronic inflammation is chronic inflammation. In 6 patients with CHF, the blood, fluid and various humoral mediators were measured during the operation, and the relationship between the two diseases was discussed. 1. Blood and serum interleukin(IL)-1 alpha, beta and tumor necrosis factor(TNF)-alpha before and after perfusion at the beginning of CHF and after 24 hours were measured. TNF-alpha of blood after perfusion at the beginning of CHF was 6 pg/ml. 2. 6-keto-prostaglandin(PG)F_<1alpha>,thromboxane(TX)B_2,11-dehydro-TXB_2 flux rates were 28- 100%, 17 - 100%, 12 -100% respectively. 3. The conversion rate of TXB_2 from 11-dehydro-TXB_2 was 38-95%, with an average of 69%. 4. 6-keto-PGF_<1alpha>, TXB_2,11-dehydro-TXB_2 and CHF_(24) have a tendency to decrease by half and to decrease by half. 5. TXB_2 +11-dehydro-TXB_2/6-keto-PGF_1 at the beginning of CHF 11.8 hours later, half of them increased and half of them decreased. The results showed that IL-1,TNF and 6-keto-PGF_<1alpha>, TXB_2 could not be detected at present, but CHF had certain influence. However, the conversion rate of TXB_2 to 11-dehydro-TXB_2 was 69% on average, and the conversion rate of TXB_2 to 11-dehydro-TXB_2 was 69% on average.
