経胸食道離断術を受け5年以上経過した肝硬変合併食道静脈瘤症例94例を対象とした。有効例と遺残再発例の術前肝機能、食道静脈瘤の状態、門脈血行動態、術中所見を比較し、また術後血管造影所見より静脈瘤遺残再発しやすい因子を検討した。94例中、術後遺残例5例、再発例12例であったが、F2以上あるいはRC陽性再発例は5例だけであった。術前ICG15分値の比較では、15分値25%以上の症例では24.6%に遺残再発をみたが、25%未満の症例では3.2%に過ぎなかった。内視鏡所見LsF3症例には26.9%の遺残再発をみたが、LsF3以外の症例では7.1%であった。術前血管造影、経動脈性門脈造影でみた左胃静脈の比較では直径6.5mm以上の症例では40.5%に遺残再発をみたが、6.5mm未満では2.2%であった。左胃静脈血流方向でみると遠肝性血流群で22.2%、他の血流群で9.7%の遺残再発であり、有意差は認めなかったが、他の血流群には術後血管造影で門脈閉塞を認めた症例が含まれていた。術中脾内圧が350mm水柱以上では24.4%、350mm水柱未満では16.7%の遺残再発率で差は認めなかった。非摘出重量は350g以上では25.5%、350g未満では8.8%の遺残再発率であった。術後血管造影では食道静脈瘤以外の側副血行路の温存が遺残再発予防に有意に作用している症例が見られた。(結論)LsF3症例、肝機能高度障害例、左胃静脈拡張例には4分の1の症例に術後遺残再発が見られたが、食道静脈瘤以外の側副血行路の温存に注意することで更に長期有効例の比率が増加すると思われた。

94 cases of cirrhosis complicated with esophageal venous aneurysm after thoracic esophageal dissection for more than 5 years Preoperative liver function, esophageal venous aneurysm status, portal blood flow, intraoperative findings, postoperative angiographic findings, venous aneurysm recurrence factors were discussed in some cases. Among 94 cases, there were 5 cases of residual disease and 12 cases of recurrent disease. The preoperative ICG score was 15 points higher than 25% in 24.6% of cases and 25% of cases without residual recurrence than 3.2%. The residual recurrence rate of LsF3 was 26.9%, and that of LsF3 was 7.1%. Preoperative angiography and arteriography compared left gastric vein diameter 6.5 mm or more in 40.5% of cases, residual recurrence 6.5 mm or less in 2.2% of cases. The left gastric vein blood flow direction was 22.2% and distal hepatic blood flow group was 9.7%. The residual blood flow group was identified by angiography after operation. Intraoperative pressure above 350 mm water column was 24.4%, and 350 mm water column was 16.7%, and the residual recurrence rate was significantly different. Non-picking weight is more than 350 g, 25.5%, 350 g, 8.8% and residual recovery rate. Postoperative angiography showed that there was no significant change in the side collateral blood pathway except esophageal venous aneurysm. (Conclusion) LSF 3 cases, liver function highly impaired cases, left gastric vein distension cases, postoperative residual recurrence cases, esophageal vein aneurysm side collateral blood circulation warm attention, and more long-term cases of increased rate.
