これまで、ラット心移植におけるアロ抗原門脈内継続投与法により移植心の生着延長を試みたが、無処置群では1週間以内に、1回門脈内注入群では20日以内に拒絶される移植心が、1週間アロ抗原を門脈内に投与することによりが200日以上拍動し続ける結果を1例であるが得ることができた。予想に反してこの実験で200日以上移植心が生着するものが1例しかなかった原因としてこのモデルの作成の困難さがあった。すなわち、外径1ミリのカテーテルといえどもラットの門脈に対してやや大きい点、1週間門脈内にカテーテルを留置するとほとんど4〜5日目で閉塞してしまう点、カテーテル留置により腸管懷死をおこすことがある点などである。一方、アロ抗原門脈内投与法と種々の免疫抑制剤との併用についてははっきりしていない。そこで、次にアロ抗原門脈内投与法についてFK506との併用により寛容の導入および移植心の生着延長効果の有無を検討した。平均生着日数は無処置群で9.6日、移植1週間前1回門注群で16.7日、免疫抑制剤であるFK506の移植前日、当日、翌日の3日間投与では43.9日、門注とFK506の併用では192.1日で、併用によりその効果は増強される結果となった。また組織学的に検討すると併用した群では軽度の細胞浸潤を認めるものの、他群ほど著明でなく、心筋の変性も軽度であった。また、併用により200日以上移植心が生着しているラットにDonorの皮膚を移植してみたところ、Third Partyより生着期間は延長するものの、3週間以内に全例拒絶され、免疫寛容状態が個体特異性でなく臓器特異性であることが示唆された。現在、リンパ球混合試験を施行しているところである。本研究でアロ抗原門脈内投与法についてFK506との併用効果を詳細に調べることにより今後さらに有効な免疫抑制法の確立に役立てたい。
