肺移植術後急性期の拒絶反応の診断に対する気管支肺胞洗浄液(BALF)の有用性について実験的に検討した。方法:雑種成犬20頭を用いてのべ10回の同種左肺移植実験を行ない、免疫抑制剤FK506(0.1mg/kg/日)投与群と非投与群に分けた。1)術前、術後1、2、3、5、7日目の健常側および移植肺のBALFを採取し、液性成分として各種サイトカイン(IL-1beta、IL-2、可溶性IL-2受容体、IL-6、TNF-alpha,IFN-gamma)についてELISA法で測定した2)胸腔鏡による肺生検により術後2、3、5、7日目の肺組織を採取し組織学的にIntercellular Adhesion Molecule(ICAM)-1とそのリガンドであるLymphocyte Function associated Antigen (LFA)-1の発現形態について検討した。結果:1)非免疫抑制群では術後2日目より7日目までにIL-1beta、IL-2、可溶性IL-2受容体、IL-6、TNF-alpha、IFN-gammaのすべてにおいて移植肺で増加を認めたのに対して健常肺では変化が認められなかった。免疫抑制群でもこれらの増加は認められなかった。その間細菌および真菌培養は陰性で、これらに起因する肺炎は否定的であった。2)胸腔鏡により通常の経気管支肺生検より多くの肺組織を得ることが可能で、しかも開胸肺生検より少ない侵襲で行い得た。術後3日目より血管内皮細胞および間質にICAM-1が発現し、周辺にLFA-1陽性Tリンパ球の集簇を認めた。3)実験系は概ね確立されたのでさらに症例数を重ねると共に、ウィルス肺炎発症時のサイトカインの推移、可溶性ICAM-1濃度の変化などの検討を行う予定である。
