日本光電社製TP-603T,AR-601G,SEN-6102Mを組合せた食道内圧及び気流測定装置を自作し、睡眠時無呼吸症候群の患者に対し、安静臥床中の食道内圧及び前鼻孔での気流の測定を試みた。睡眠時無呼吸症候群の診断はアプノモニター(チェスト社製)及びパルソックス8(アムコ社製)によって行った。バルーンカテーテルを頚部食道内に留置して圧トランスデューサーとし、前鼻孔直前にサーミスターを留置してこれを速度トランスデューサーとした。サーミスターの温度補正は腋下に補正用サーミスターを留置して行った。測定の結果、食道内圧は高値を示したが、正常者との有意差は明らかではなかった。食道内圧と睡眠時無呼吸症候群の重症度との相関は見られなかった。前鼻孔での気流は全般的に安定した結果が得られなかった。特に左右どちらの鼻孔を用いたかによっての差が大きかった。これは睡眠時無呼吸症候群患者の病態を反映した可能性が高いが、測定にサーミスターを用いたための機械的な問題も関与していたと考えられる。筆者は過去の研究において睡眠時無呼吸症候群患者の鼻腔通気度がアドレナリンによる鼻処置で改善する程度が正常者よりも大きいことを報告しているが、このような大きな改善は食道内圧に関しては認められなかった。この結果、鼻処置のみでは睡眠時無呼吸症候群の病態の全般的な改善は得られない可能性が示唆された。各種手術の術後における食道内圧等の改善傾向の有無については現在検討中である。
