【はじめに】声帯麻痺治療の新しい試みとして形成外科領域において使用されている自家吸引脂肪の麻痺声帯内の注入を行い、良好な結果を得たので報告する。【対象と方法】対象は、頚部迷走神経鞘腫術後の62歳男性(症例1)と甲状腺腫瘍術後の72歳男性の2症例(症例2)であった。1、脂肪の採取:脂肪採取用の鈍針を注射器に装着し、持続陰圧をかけながら下腹部より脂肪を採取した。2、脂肪の精製:採取後、脂肪から血球成分や組織液成分を除去するために抗生剤入りの生理食塩水で3〜4回洗浄した。3、脂肪の注入:甲状軟骨下縁から経皮的に14G針を用いて傍声帯間隙内へ脂肪組織を注入した。注入時は、直接喉頭鏡下に喉頭顕微鏡を用いて注入部位を観察し、適切な注入部位を決定した。【結果】1、自覚的な改善度:症例1、2とも自覚的に発声持続時間(MPT)の延長や声の強さの増強、発声音域の拡大を認め、日常生活のコミュニケーションにおいて著明な改善を認めた。2、他覚的検査:(1)MPT;症例1は術前にMPTが1秒未満であったが、術後5ケ月目には18秒と正常範囲に改善した。症例2は術前に5秒であったが術後3ケ月の時点では、10秒とMPTの延長を認めた。(2)音声評価(リオン製音声評価装置SH-10による);PPQ、APQ、NNEについて各々計測し、音声の評価を行った。症例1は、術前はMPTが1秒に満たず、計測できなかった。しかし術後1ケ月目には、「軽度の嗄声」、5ケ月目では、「正常」にまで改善した。症例2も術前の評価は、「中等度の嗄声」であったが、術後3ケ月目には、「正常」まで改善した。【まとめ】2症例とも術後の観察期間が短いため脂肪注入の評価を下す事は困難である。しかし自家組織の移植であるため合併症等も認められず、有効な方法であると考えられる。今後、各症例の長期に渡る術後経過の観察と症例数の蓄積が評価を下す上で重要であると思われる。

Refining of fat: After collection, remove the fat, blood cell components and tissue fluid components, add antibiotics and put them into physiological diet water and wash them 3 to 4 times. 3. Fat injection: A 14G needle is inserted into the skin under the thyroid cartilage and the fat tissue is injected into the gap between the thyroid cartilage and the thyroid cartilage. When injecting, the injection site is inspected directly under the laryngoscope and the laryngeal microscope is used, and the appropriate injection site is determined. [Results] 1. The transplantation of one's own tissue, the complications, etc., are recognized, and the effective method is recognized. From now on, it will be important to review the long-term and post-operative effects of each case and the accumulation of symptoms and the number of cases.
