慢性副鼻腔炎が鼻内手術だけでコントロールが可能か、またはCaldwell-Luc法等の鼻外法を併用しなければならないかは、上顎洞病変を鼻内法でどこまでコントロールできるかに係っている。そこで今回この上顎洞病変に注目し術中の内視鏡下の上顎洞病変を次のように分類した。すなわち上顎洞の粘膜の厚さを粘膜表面の状態と血管の透見度から(1)Mild(2)Moderate(3)Severeの3段階に分類しさらに上顎洞の内容物として貯留液、Polyp、Cystの存在有無についても検討し、それぞれの頻度、術後の治癒過程、及び術後の症状の改善率について比較検討した。対象は115例、219側である。Mildタイプが99側45%、Moderateタイプが98側45%、Severeタイプが22側10%であった。これらのタイプ別に貯留液、Polyp、Cystの存在率を見ると貯留液はMildで30%、Moderateで63%、Severeでは73%であった。PolypはMildで11%、Moderateで8%、Severeでは1例も認めなかった。またCystはMildで14%、Moderateで8%、Severeでは1例も認めなかった。以上から貯留液は粘膜が厚いほど存在率が高く、PolypやCystは粘膜が薄い症例で存在率が高かった。PolypおよびCystの予後は術後1年が経過した時点で判定した。その結果Polypは19例中4例、Cystは22例中5例に残存を認めた。手術成績は良好であるが、粘膜の厚さ別の予後を判定するため術後に鼻漏、後鼻漏が残存する症例を各病態別に比較した。その結果Mildで13%、Moderateでは15%であるのに対し、Severeでは31%に症状の残存を認めた。症状の残存率を内容物別で比較すると、貯留液症例が20%で、PolypやCystは10%以下であった。以上をまとめると粘膜が厚いほど、または貯留液が存在するほど予後が悪かった。これら病変程度以外に治癒過程に影響する因子として手術手技、術後管理、全身疾患 (喘息、鼻アレルギー、糖尿病等)、副鼻腔の発育程度等が考えられた。しかしその他の大部分の症例は治癒しており、慢性副鼻腔炎の初回手術は全例、内視鏡下鼻内手術でコントロールが可能と考えられた。
