口唇裂口蓋裂患者の補綴処置においては、外科及び矯正治療により改善された歯及びsegmentのrelapseを防ぐ配慮はもちろんのこと、補綴処置終了後の口腔内環境を長期に渡り健康に保つために、顎裂を含み脆弱な骨組織を支持組織とする上顎の支持能力を高め、下顎との咬合力負担能の差を可及的に小さくする配慮が必要である。この二点から、補綴処置においては、顎裂をはさむ数歯ずつの連結固定が必須である。すでに我々は、片側性唇顎口蓋裂患者を対象とし、咬合力負担能という観点から適正な連結範囲を決定する基準を見出す目的で、連結範囲を変え、咬合力を測定し、連結範囲が咬合力負担能におよぼす影響について検討を加えた。その結果、咬合力負担能という観点からは、連結範囲は、顎裂をはさんで2歯ずつで充分であり、それ以上延長しても効果を持たないことが明らかとなった。本研究においては、両側性唇顎口蓋裂患者を被験者とし、ブリッジによる補綴処置を想定し、連結範囲を変え、咬合力を測定した。その結果、以下の結果を得た。1.連結範囲を拡大すると咬合力は増大する傾向にあったが、右側での連結範囲が犬歯1歯のみの場合では、左側で連結範囲を拡大しても咬合力の有意な増大はみられない場合が多かった。2.連結範囲を顎裂をはさみ2歯以上としても咬合力に有意な増大はみられず、咬合力負担能に対する連結の効果は期待できないと考えられた。3.両側性唇顎口蓋裂症例は、片側性唇顎口蓋裂症例に比べ、中切歯部において連結による咬合力負担能増大の効果が少ない傾向にあった。
