本学歯学部1年生40名に対して、まず、顎間節症状(顎間節雑音,疼痛,顎機能障害等)の自覚症状についてのアンケートおよび同一検査者1名の聴診による顎関節雑音の有無および性状についての診査を行った。アンケートの結果では、11名(約28%)が関節雑音を自覚し、残りの29名(約72%)は関節雑音を自覚していなかった。しかし、聴診による客観的診査では19名(約48%)に関節雑音が認められた。つまり、自覚していないが関節雑音を有している者が8名(約20%)いた、顎関節部の疼痛に関しては、全員なしの回答であった。開口障害については、3名(約8%)が訴えていたが、診査では全員38mm以上あり、顕著な開口障害は認められなかった。また、4名(10%)が自分のかみあわせに異常感を訴えていた。聴診によって認められた関節雑音の性状については、reciprocal click,eminence click crepitusに分類したが分類できない微妙な性質の雑音も認められた。関節円板前方転位に起因した典型的なreciprocal clickは1名にしか認められなかった。eminence clickが13名と多いのが特徴的であった。したがって、本学歯学部1年生は約半数に顎関節雑音を有しているものの疼痛は無く、関節円板前方転位といった進行した病態のものは少ない結果であった。次に、客観的診査により関節雑音が認められた者に対し、印象採得を行い研究用模型を作製した後に当教室で開発された非接触型下顎任意点運動測定装置を用いて下顎運動測定を行った。咬合状態は歯列不正のものが多かった。下顎運動は、顆頭運動の3次元動態、顆頭の回転と移動との関係に注目した分析を行い、関節雑音の性状と関連づけてその特徴を研究中である。今後は顎関節雑音と臨床症状および下顎運動の経年的変化を分析していく予定である。
