项目摘要

本研究は,昨年度に引き続いた経年的な研究であるので,調査対象は昨年度と同じ対象で本学歯学部2年生40名とした。アンケートの結果では,19名(約48%)が関節雑音を自覚していた.聴診による客観的診査でも19名で一致していた.保有者の数は昨年と同じであったが,新たに保有者となったものが1名,消失した者が1名いた.関節雑音の性状については,昨年は分類できなかった微妙な性質の雑音に関してもドップラー聴診装置により記録することが可能となった。結果は,reciprocal clickが2名,eminence clickが12名,crepitusが2名,single clickが3名であった.昨年からの変化は,eminence clickからreciprocal clickに変化した者が1名,single clickが新たに発生した者,消失した者がそれぞれ1名ずついた.single clickは,reciprocal clickに移行することも消失することもあることがわかり,初期症状として重要であると考えられた.顎関節部の疼痛に関しては,single clickからreciprocal clickに変化した者1名に認めた.開口障害については,crepitusの2名に認めた.ただし,顕著な開口障害ではなかった.非接触型下顎任意点運動測定装置による下顎運動測定の結果は,臨床症状の変化のある者で下顎運動の変化が大きく,症状の変化のない者は下顎運動の変化も少なかった.single clickの顆頭運動はクリックに対応して小さい軌跡の変化が認められた.single clickからreciprocal clickに変化した者と消失した者との昨年の顆頭運動の違いは,閉口末期の顆頭の回転と移動との関係が前者でやや移動優位の程度が大きかった.single clickが消失した者の今年の顆頭運動は閉口末期の顆頭の移動が正常者の平均と比較してやや大きかった.eminence clickの顆頭運動は左右の協調性の悪いことが特徴であったが1年間の変化は小さかった.crepitusの顆頭運動も1年間の変化は小さかった.来年も経年的変化を分析し,特にsingle clickに着目して検討を行なう予定である.
