近年、不正咬合に伴う咬合異常が顎機能に悪影響を及ぼす可能性が示唆され、歯科矯正学における治療は、審美性のみならず三次元的な下顎位および下顎の機能的運動を考慮した咬合の再構成を行なっていく必要がある。本研究の目的は、顎機能正常者ならびに不全者の下顎頭運動経路を検討し、その結果を不正咬合の診断・治療に積極的に取り入れ、審美的にも機能的にも良好な咬合を構築するための治療体系を確立していくことにある。そこで本研究では、顎機能解析機器の一つであるcomputer aided diagnostic axiograph version 2.0(CADIAX ver.2.0)を用い、顎機能正常者群(33名)ならびにMRI検査により片側もしくは両側に復位性の関節円板転位の認められる顎機能不全者群(27名)の習慣性開口運動に着目し、その下顎頭運動について検討した。その結果、1.顎機能正常者群と顎機能不全者群の習慣性開口運動における下顎頭の最大側方偏位量を比較したところ、顎機能不全者群で有意に大きかった。2.顎機能正常者群と顎機能不全者群の習慣性開口運動における各群それぞれの左右下顎頭の最大前方滑走量を比較したところ、有意な差は見られなかった。3.顎機能正常者群と顎機能不全者群の習慣性開口運動における左右下顎頭の滑走量の同調性を比較したところ、顎機能不全者群において有意に同調性が乱れていた。4.顎機能正常者群と顎機能不全者群において、習慣性開口運動時の最大側方偏位量と左右の下顎頭の滑走運動量の同調性の相関関係を比較したところ顎機能不全者群において、同調性の不調和と最大側方偏位量との間に有意に正の相関関係が認められた。以上のことから、顎機能不全者は顎機能正常者に比べ下顎頭の運動範囲が拡大されていることが示唆された。
