ヒトgamma-glutamyl transpeptidase(gamma-GTP)遺伝子の多形性については、5′非翻訳領域において胎児、HepG2細胞、および胎盤のそれぞれに相当する3型のあることが明らかにされている。そこで、アルコール(Al)性臓器障害の発現に関与する遺伝的素因をgamma-GTPの遺伝子多形性の面から検討することとした。対象はAl性肝・膵障害患者8例、非Al性障害患者7例、正常者は10例で、剖検ないしは手術時に得られた肝組織からgamma-GTP遺伝子型の分析を行った。遺伝子型の検索は、各3型に共通する領域をはさむprimerを設定し、常法に従ってRT-RCRを行い、胎児型、胎盤型に特異的なoligoprobeを用いてSouthern blot hybridizationを行い、決定した。その結果、それぞれ各疾患に特有な遺伝子型はなく、正常肝で認められた胎盤型単独の1例(4%)を除きすべて混合型であった。胎児型とHepG2細胞型の混合は5例(20%)、HepG2細胞型と胎盤型の混合は4例(16%)、胎児型と胎盤型の混合は7例(28%)で、3型の混合が8例(32%)に認められた。さらに、肝細胞癌患者3例についても分析を行ったところ、非癌部組織では、3例中2例で混合型、1例でHepG2細胞型であったが、癌部組織ではいずれの例でもHepG2細胞型のみが単独に認められた。そこで、胎児、HepG2細胞、胎盤のそれぞれの型の発現状況をさらに細かく分析するため、新たに、各遺伝子型を的確に検出できるprimerと各型に特異的なprobeを作成し、Al性肝・膵障害患者の肝組織とともに、肝細胞癌患者の非癌部と癌部の肝組織も検討に加え、発現型と発現量について目下検討中である。これらの検討から、gamma-GTPのアルコールによる誘導と臓器障害の関係、さらにはgamma-GTPの遺伝的多形性と肝細胞癌の発生との関係がより明らかになると考えられる。
