FACES-IIIを用いて臨床的問題を抱えた家族の凝集性と適応力を測定した。【研究方法】対象者は都内の精神科外来を過去3年間に訪れたクライエントとその家族で、13歳から22歳の思春期の子どもがいる家族である。使用した質問紙はオリジナルのFACES-IIIを翻訳したものを用いた。翻訳に際してはBack transtationによって訳出による誤差を最小限に留め、また約1000人に対するpilot studyによってその妥当性と信頼性を確認した。対象となる912家族に各5部ずつ調査用紙を郵送により配布し、同居している家族全員に調査を依頼した。その結果148家族より回答が得られ(回収率:16.2%)、うち13歳から22歳の子どもがいる家族のメンバー342名を分析の対象とした。また、対照として中・高等学校とボランティア組織を通じて郵送により同年代の子どもを含む家族に調査を依頼した。【結果】臨床家族の凝集性は順にEnmeshed4.5%(対照群14.3%),Connected20.4%(28.2%),Separeted32.7%(30.8%),及びDisengaged42.4%(26.8%)と、対照と比較して凝集性が低い傾向にあった(chi^2<.01)。また、適応力は順にChaotic37.7%(対照群40.6%),Flexible39.0%(39.7%),Structured18.4%(14.7%),及びRigid4.9%(5.1%)であり、対照と比較して有意な差が見られなかった(chi^2>.05)。これらは性別、世代別に分けて比較しても同様な傾向が見られた。【考察】精神科的問題を抱える思春期家族は凝集性が低い傾向にあり、これは心身症家族に見られるenmeshed familyとは異なる結果となった。臨床的には母子間などに強く凝集した関係に多数遭遇するが、家族全体で見ると、むしろ遊離していることがわかる。臨床的妥当性は凝集性尺度に関しては検証されたが、適応力については臨床家族と対照家族に有意差が見られず、検証されなかった。
