成犬8頭を対象とし、PEEPOcmH_2O、5cmH_2O、10cmH_2O、15cmH_2O,15cmH_2O+輸液負荷の条件下で、左室圧-容積曲線を作成し、PEEPの心収縮能、拡張能への影響と輸液の効果について検討を行い、次のような結果が得られた。1.PEEPにより、前負荷は減少傾向を示し、後負荷は増加する傾向を示した。2.PEEPによる心収縮能の変化としては、stroke volumeの有意な減少が認められた。3.PEEPにより時定数は有意に減少し、その原因として前負荷の減少の影響が考えられた。4.PEEPによりスティフネス係数は延長傾向を示した。その原因として、PEEPによる胸腔内圧の上昇により心臓が外側から圧迫を受けることや、肺血管抵抗の上昇により、右室の後負荷が上昇し、右室の充満圧が高くなることにより心室中隔の偏位が生じ、左室が圧迫されることなどが考えられた。5、通常治療として行われるより少量の輸液(ハイドロキシエチルスターチ2ml/kg)により、時定数およびスティフネス係数は改善する傾向を示した。

8 adult dogs were tested for PEEP OcmH_2O, 5cmH_2O, 10cmH_2O, 15cmH_2O, 15cmH_2O + infusion load. The left ventricular pressure-volume curve was constructed. The effects of PEEP on cardiac contractility and tension energy and infusion efficiency were discussed. 1. PEEP shows a tendency to decrease in front load and an increase in back load. 2. PEEP is the most important factor in the reduction of stroke volume. 3. PEEP time is intentionally reduced, and the reasons for this decrease are discussed. 4. PEEP is the most important factor in the development of the disease. The causes include: increase in intrathoracic pressure due to PEEP; increase in pulmonary vascular resistance due to pressure on the lateral side of the heart; increase in afterload due to right ventricle; increase in right ventricular filling pressure due to deviation of the ventricular septum; increase in left ventricular pressure due to pressure on the lateral side of the heart. 5. The tendency of small amount of infusion (2ml/kg) to improve the frequency and quality of treatment was shown.
