项目摘要

閉塞性睡眠時無呼吸症候群の発症要因の一つに、上気道の解剖学的異常がある。その治療法の一つである口蓋垂軟口蓋咽頭形成術Uvulopalatopharyngoplasty(UPPP)は主に、中咽頭レベルの狭窄に有効な手術法と考えられるが、その手術を行なった場合、一般に有効率は50ないし60%と言われている。現在、我々術者にとって最大の関心事は、いかに的確な閉塞部位診断を術前に行ない、手術効果の予測性を把握し、適切な治療法を選別するかであり、そのためのセファロメトリー(咽頭顔面×線規格写真)の有用性について報告してきた。従来のセファロメトリーは各測定点間の距離と角度との検討であったが、本研究は、術前のレントゲン側面像について軟口蓋、口腔、舌、咽頭の側面積の測定を行ない、術前の治療効果の予測および術後の面積の評価を二次元的な検討で行なった。対象は、睡眠時呼吸障害、昼間の睡眠傾向、あるいは、いびきを主訴とする成人患者である。セファロメトリーを施行し、レントゲン写真をトレースして、スキャナーで画像を取り込み、MacintoshコンピュータでFlexl Trace1.02を使用し軟口蓋、口腔、舌、咽頭の側面積を測定した。各測定値について平均と標準偏差を求め、異なる患者群間でf検定による統計学的検討を行った。これらの結果をもとに、解剖学的異常の把握と、術後の治療効果の検討を行なった。

The development of respiratory syndrome during sleep is due to an abnormality in the anatomy of the upper respiratory tract. Uvulopalatopharyngoplasty(UPPP) is a surgical procedure for the treatment of throat stenosis. It has an effective rate of 50% to 60% in general. Now, the greatest concern of our surgeons is to report the usefulness of diagnosis of the affected area, preoperative diagnosis, predictability of surgical results, selection of appropriate treatment methods. The distance and angle between the measurement points were discussed. The study included the measurement of soft flap, oral cavity, tongue and throat area on preoperative bottom image, the prediction of preoperative treatment effect and the evaluation of postoperative area. For example, breathing disorders during sleep, daytime sleep tendencies, and adult complaints. The soft flap, oral cavity, tongue and throat area were measured using Flexl Trace 1.02. Statistical evaluation of mean and standard deviation for each measurement and determination of differences between patient groups The results of the study, anatomical abnormalities and postoperative treatment results were discussed.
