高齢者の増加にともない、高齢者の外科的処置の頻度も増えてきている。特に女性においては骨粗鬆症に罹患している人も多く見受けられる。創傷治癒を比較すると、骨粗鬆症に罹患している患者は、一般的に悪い傾向にあり、高齢者の社会復帰を考える上で重要である。近年、骨組織には、骨芽細胞の機能を調節するtransforming growth factor- β(TGF- β)や骨のリモデリングに関与しているとされているbone gla protein(BGP)が存在することが明らかにされている。したがって、卵巣摘出動物を実験的に骨折させ、骨折後のTGF-βおよびBGPを測定することは、骨粗鬆症罹患者の骨折あるいは抜歯窩の治癒を考える上で重要である。そこで、骨折後の骨組織中のTGF-βおよびBGPの変化を経時的調べ健常動物との比較検討を行った。12週令のラットを2つに分け卵巣摘出術(OVX)を施すOVX群と何も施さないN群に分け、その後3か月間通常飼育する。そしてそれぞれのラット大腿部(片側)を人工的に骨折させ、0、3、7、12、17、22、28、34日にそれぞれ屠殺する。直ちに、両側の大腿部を取り出し、骨組織中のTGF-β,BGPを測定する。また一部は標本を作成して組織学的検索を行った。TGF-βおよびBGPの測定は摘出した大腿骨を脱水、脱脂し、粉砕して骨粉を作成する。この骨粉を0.5N塩酸グアニジンで抽出しこれを酸に溶解し、TGF-βはTGF-β_1 ELISA、BGPはRIA法を用いて測定した。その結果、TGF-βはNおよびOVX群ともに骨折後7日目から上昇し22日目よりプラトーになった。OVX群ではその値が低値を示した。BGPはOVX群はN群に比べ低値を示した。組織学的には、N群では28日目にはほとんど骨折部が接合していたのに対しOVX群では、偽関節を形成しているものが3割程度認められ、全体的に骨形成の程度が低かった。これらのことから、骨形成にはTGF-βおよびBGPの関与がうかがわれた。今後さらにほかの因子について検索を行う予定である。
