项目摘要

急性間欠性ポルフィリン症(AIP)の原因は、ヘム合成経路の3番目の酵素であるPorphobilinogen deaminase(PBG-D)の活性が約半分に低下することであるが、さらに薬物、飢餓、妊娠等のストレスが加わることにより初めて発症する。従って、酵素異常を持ちながら発症しない、潜在者、が多数存在する。本邦における本疾患の頻度は、人工10万対0.3〜3.0人という報告があるが、多くの診断されずにいる潜在者が存在すると考えられており、実際は報告された頻度の10倍ぐらいあるという推定もある。これらの存在者を正しく診断し、その旨を知らせて、先に述べたような誘因をさけるようにアドバイスすることは、発症を予防するために非常に重要である。従って、潜在者を確実に診断することは、患者の診断を正確に行うことと同様に非常に重要であるが、現在一般的に行われている赤血球におけるPBG-D酵素活性の測定等の種々の方法では確定診断に至れない例もあり、遺伝子の異常を調べ、それにもとずいて潜在者の診断を確実にすることが望まれる。我々は、本遺伝子異常を調べる研究を以前より行っており、奨励研究Aによる補助を受け、それを一助として、さらに研究を進めた。その結果、我々がこれまでに報告してきた2種類のPBG-D遺伝子異常(その内の1種類は1994年に印刷された、研究発表1)とは別の本遺伝子異常を新たにみつけた(投稿中)。さらには、その患者家系の人達に、この点変異が有るかどうかをPCR法で増幅した染色体DNA断片が制限酵素HhaIで切断されるかどうかにより判断するという簡便な方法を用いて調べ、4名の潜在者を新たに見つけることができた。本点変異はオランダにおいて高頻度(X-F Gu et al.1993:調べた49家系15家系で本異常を認めた)で認められる異常であり(創始者効果)、本邦でもオランダでみられたと同様に高頻度に認められる可能性もあるので現在他のAIP家系で本異常が認められるかどうかをスクリーニング中である。これまで行ってきた手法を引き続き行うことにより、さらに多くのAIP家系において遺伝子異常を新たに見つけ、それによる家系調査により更に多く潜在者を見つけることが出来ると思われる。

The causes of acute intermittent pancreatitis (AIP) include the decrease of Porphobilinogen deaminase(PBG-D) activity in three enzymes of the synthetic pathway. Abnormality of enzymes persists, and symptoms, potentials, and majority exist. The frequency of this disease in this country is 100,000 people, 0.3 ~ 3.0 people, and the frequency of reporting is 10 times that of reporting.
