人口の高齢化が進む今日、有床義歯を用いた補綴処置を行う頻度が高まっている。特に、全部床義歯の必要な患者の多くは高齢者であり、一般に新しい環境に対する順応性が低下している。このような患者が新しい義歯を受入れやすくするためには、義歯装着が床下粘膜へ及ぼす影響について、特に義歯機能時の挙動を考慮した情報が必要である。本研究は、荷重前後の床下粘膜の抹消血液循環の変化を、粘膜の特に薄い口蓋正中部を中心にレーザードップラ血流量計を用いて計測し、さらに当教室で従来より行われているBモード超音波診断装置による計測を同一部位に加え、義歯装着による床下粘膜への影響とその対応策について検討することを目的とする。本研究においては、実験義歯が血流量の変化・床下粘膜厚径計測のテンプレートを兼ねているため、以下の点に注意して慎重に製作した。前歯部からプローブの挿入が可能となるように臼歯部のみに咬合堤を付与し、床用レジンは適合性に優れているイントプレスを用いた。荷重量を最大咬合力に規定できるように下顎実験義歯の左右第1大臼歯相当部に小型荷重センサを設置し、センサの受圧部のみで上顎実験義歯と咬合するように調整した。また、粘膜表層の血流量は温度変化に大きく影響されるので、実験義歯口蓋部に温度センサを組み込み粘膜表面の温度をモニタリングできるようにした。同一部位を繰り返し計測できるように上顎実験義歯の各計測部位にはプローブホールを設けた。この実験義歯を用いて測定をおこなった結果、安静時粘膜厚径に対する粘膜沈下量の割合(粘膜沈下率)が、12.7〜15.1%と他の部位より高い値を示した口蓋正中部を中心とする範囲で、荷重前後の血流量の変化が大きい傾向が示された。また、この範囲において除重時の粘膜変位の変位回復比が他の部位より小さいことが明らかになった。これらの結果は義歯機能時にリリ-フすべき範囲と量を決定する判定基準となる。

The range of blood flow before and after load is large. In addition to the weight of the mucosa, the position of the mucosa is different from that of the other parts. The result is a decision criterion for determining the time limit of the function.
