本研究では、患者の視点からみた、外来診療における医師・看護婦・患者の3者間のコミュニケーションの構造を調査・分析した。研究上これまでの知見・文献検討・事例調査により、(1)中心的因子:(1)診療業務における情報提供構造・治療検査決定までのプロセス→(2)納得・信頼構造→(3)成果としての生活状況(医師の指示遵守・生活コントロール) (2)(1)に作用する内的因子:個人(性・年齢・健康状態),家庭(家族に対する責任),職(職業) (3)(1)に作用する外的因子:場(診療科特性),接触(経過時間・対応時間・入院経験)という構造を形成していることが予測された。この構造をもとに調査票を作成し、外来診療における様々な要素が存在するある大学病院の外来患者全員(自己記載可能な成人)に対し、調査を実施した(配布804・回収770〈回収率95.8%〉・有効回答734〈未成人36名削除〉)。研究上の重要な因子である「医師への信頼」「看護婦への信頼」に対して、より影響力の強い変数を、数量化理論第II類を用いて上記(1)(2)(3)から抽出したところ、前者の寄与率0.60(24数)、後者の寄与率0.55(24変数)を得た。これにより予測された因子の構造が、実構造に比較的近似していることが推測された。「医師への信頼」に強く影響する因子の最上位は「看護婦への信頼」であり、続いて「満足する治療説明」「治療検査への納得」「今の健康状態における生活のやり方の理解」「医師の傾聴姿勢」であった。「看護婦への信頼」に強く影響する因子の最上位は「医師への信頼」と「看護婦に対する患者の傾聴姿勢」であり、続いて「看護婦の患者理解」であった。本解析を通じて、「医師の信頼」と「看護婦の信頼」の間には強い相互作用が存在し、提供した医療サービスの成果(健康状態の改善・生活コントロール)を得るために必要と判断される患者との信頼関係を形成する上で、医師・看護婦両者に対する信頼が重要であることが示唆された。

This study investigated and analyzed the structure of the relationship between the patient's viewpoint and the external diagnosis and treatment of the doctor, nurse and patient. In the above study,(1) central factors:(1) information supply structure in medical service, selection of treatment evaluation decision,(2) acceptance and information structure,(3) results and living conditions (doctor's instruction compliance, living conditions),(2)(1) internal factors: individual (sex, age, health status), family (family responsibility), occupation (3)(1) external factors: field (medical characteristics), contact (elapsed time, response time, hospital admission), and formation of internal structure. The structure of the survey ticket was created, and the factors of external diagnosis and treatment were present. All external patients in the university hospital (self-recorded adults) were investigated (distribution 804, response 770, response rate 95.8%, response 734, elimination of 36 adults). The important factors in the study are: "physician's letter" and "nurse's letter". The influence of the two factors is strong. The quantitative theory is used in the second category. The former has a rate of 0.60(24), and the latter has a rate of 0.55(24). The structure of the prediction factor is similar to that of the comparison factor. The highest level of the strong influence factors is "nurse's letter","treatment instructions","treatment investigation","understanding of life's current health status" and "doctor's inclination posture". The highest influence factors of "nursing woman's trust" are "doctor's trust","nursing woman's inclination posture" and "nursing woman's patient understanding". Through this analysis, there is a strong interaction between the "trust of doctors" and the "trust of caregivers", and the results of providing medical services (improvement of health status and daily life) are achieved. This shows that trust between doctors and caregivers is important in forming a trust relationship between patients and patients that is necessary and judgmental.
