看護職が患者のケアニードの解決に際し,社会資源を有効に活用するための基礎的資料を得る目的で,京都府内20カ所の病院施設に勤務する看護職3470名を対象に,社会福祉に関する意識についてアンケート調査をした.調査期間は,1995年11月15日〜12月31日であった.回答を得た2695人(回収率77.7%)のうち,年齢・性別が判明している2651人(98.4%)を解析対象とした(女性99.8%,平均年齢31.7±9.3(19〜63)歳),質問内容は,対象者の属性,社会福祉に対する関心度,看護学生時代の社会福祉の講義,福祉制度・福祉施設・福祉施策等の認知度および活用度であった.免許取得別構成は,看護婦(士)84.7%,准看護婦(士)8.1%,保健婦・助産婦7.0%であり,経験年数は,1〜3年27.6%,4〜9年32.9%,10〜19年24.0%,20年以上15.3%であった.また,職位は,管理職(看護部長・婦長・主任等)14.0%,臨床指導者・看護婦(±)85.4%であり,設置主体別構成は,都道府県26.3%,市町村22.1%,日赤19.7%,社会福祉法人7.0%,民間病院5.6%,国立5.2%,済生会4.0%,学校法人3.8%,医療社会法人3.5%,共済組合2.8%であた.(1)ゴールドプランやエンゼルプランについて,「知らない」回答した人は過半数以上であった.(2)経験年数別社会福祉への関心度は,「ある」と回答した人は,20年以上が52.0%で最も多く,1〜3年では16.3%であった.経験件数が増すほど関心度が高くなる傾向が認められた(p<0.01).(3)職位別関心度は,「ある」と回答した人は,管理職49.2%,スタッフ23.3%であり,管理職者で比率が有意に高くなっていた(p<0.01).(4)設置主体別関心度では,「ある」と回答した人の比率は医療社会法人38.7%,国立36.3%,市町村30.6%,済生会30.5%の順に高かった.(5)福祉制度・施設・サービスの15項目の認知度について,「よく知っている」の上位3項目は,高額療養費11.0%,身体障害者運賃割引制度9.7%,ディサービス9.6%であり,「知らない」の上位3項目はライトハウス43.5%,更生医療給付制度32.7%,在宅介護支援センター32.4%の順であった.管理職および経験年数が増すほど,福祉制度やサービスの認知度が高い傾向にあった.以上から,医療・看護と密接な関わりのあるゴールドプランなど,我国の福祉施策に関する知識の普及,特に経験年数の少ないスタッフに対する社会福祉への啓蒙の必要性が示唆された.
