1.目的:本研究では,左半側空間無視患者に単語を音読させ,読み誤りのパターン,neglect dyslexiaと半側空間無視との関連について検討することを目的とする.2.方法:対象は東京都老人医療センターに入院ないし通院中の右利きの右半球損傷患者(全例脳硬塞)で,発症時に顕著な左半側空間無視を呈した5例である.刺激語は「電子計算機による新聞の語彙調査(IV)」の長単位表(国立国語研究所,1973)から100語を選択した.刺激呈示にはブック型コンピュータを用い,呈示時間が制御できるようにした.練習試行において,各症例とも正答率が50〜60%程度となる呈示時間を,文字種と単語長ごとに求めておき,それを本試行の呈示時間とした.実験にあたっては,東京都老人総合研究所と東京都老人医療センターの各倫理委員会の承諾を得た.また,患者に本研究の趣旨を十分に説明し,併せて本実験は診療以外の研究目的で行なわれる旨を説明した上で,参加の同意を得た.3.結果:読み取りを分析した結果,4例にはneglect dyslexiaが認められたが,1例ではneglect dyslexiaが認められず,無視とneglect dyslexiaが乖離する場合のあることが示唆された.一方,neglect dyslexia4例のneglect errorには,(1)漢字、かなの差がない,(2)半盲があると左の文字の見落としの比率が高くなる,(3)単語の文字位置に関係なく,左半分の文字全部をほぼ読み誤り,単語長の影響を受けない,ことがわかった.この結果は,今回のneglect dyslexia4例の読み誤りが,単語認知過程の初期段階の障害よって生じた可能性が高く,半側空間無視により説明できることを示唆するものである.
