瀬川病は,1976年瀬川らにより報告され,四肢のジストニア,深部反射亢進,歩行障害,パーキンソニスムなどを主症状としレボドパ製剤が著効する家族性の神経疾患である.近年,第14番染色体長腕にあるGTP cyclohydrolase I (GCHI)遺伝子の異常により発症すると考えられている.われわれは宮城県岩出山地方において本疾患を疑う1家系を発見し,すでに報告されているGCH I遺伝子の異常の有無について検討を行った.患者白血球よりDNAを抽出,GCH Iのexon 1からexon 6をそれぞれPCR法により増幅し(exon 1についてはsubcloningを行った), Sanger法に基づきDirect sequenceにより塩基配列を確認し,正常対照,既報告の遺伝子異常と比較した.その結果,患者遺伝子には明らかな異常は認められず,今回の検討では本患者には瀬川病に特有とされる遺伝子異常を検出できなかった.しかし,我々の患者は神経学的に瀬川病と類似しており,また,明らかな家族性の異常を呈していることから,GCH I酵素活性や,類似の他疾患の遺伝子検索など,さらなる検討が必要と考えた.

Segawa disease was reported in 1976. It is a familial neurological disorder in which the limbs are affected, deep reflexes are hyperactive, walking is impaired, and the main symptoms are abnormal. In recent years, GTP cyclohydrolase I (GCHI) gene on chromosome 14 has been found to be abnormal. 1 family of suspected disease was found in Miyagi Iwayama area, and the report was carried out on the presence or absence of abnormalities in GCH I gene. DNA extraction of leukocytes from patients,GCH I exon 1 and exon 6 were amplified by PCR (exon 1 subcloning), Sanger method was amplified by DNA Direct sequence, normal contrast, both reported DNA abnormalities were compared. As a result, the patient's genetic abnormalities were detected, and this time the patient's genetic abnormalities were detected. The patient's neurologic disease is similar to that of the family, and the GCH I enzyme activity is similar to that of other diseases.
