軟骨無形成症における、線維芽細胞増殖因子受容体(FGFR)-3の、膜貫通領域の突然変異同定が報告されて以来、次々と軟骨異形成におけるFGFRの異常が明らかとなってきている。私は、現在までに当科外来通院中の軟骨無形成症40例、軟骨低形成症15例、Pfeiffer症候群1例、偽性軟骨無形成症1例、致死型四肢短縮症1例のFGFR-3膜貫通領域、イミュノグロブリン様領域IIIの後半について、PCRを用いた多型解析とRNase protection法を用いたスクリーニングにより、突然変異の検出を試みている。その結果、軟骨無形成症のほぼ全例に共通の突然変異を検出し得たが、他の症例ではこの領域には変異を見いだし得なかった。さらに、外科的治療適応となった2例において骨芽細胞の培養を行い、本細胞系におけるFGFR-3の発現をRT-PCRにより確認した後、FGFR-3のリガンドである酸性FGFの作用を十分に伝達し得ないことが明らかとなった。今後、FGFRの変異の細胞内情報伝達機構の障害の詳細を同系で検討するとともに、培養軟骨細胞に変異FGFR-3を導入し、軟骨細胞増殖におよぼす影響を検討する予定である。

Since the report of sudden changes in the receptor for fibroblast growth factor (FGFR)-3 and membrane penetration in achondroplasia, the abnormality of FGFR has become apparent in the second and third stages of cartilage dysplasia. In addition, there were 40 cases of achondroplasia, 15 cases of hypochondrogenesis, 1 case of Pfeiffer syndrome, 1 case of pseudoachondroplasia, 1 case of fatal limb shortening, 1 case of FGFR-3 membrane penetration domain, 1 case of FGFR- All cases of achondroplasia were found to have sudden changes, while other cases were found to have sudden changes. In 2 cases, the development of FGFR-3 in this cell line was confirmed by RT-PCR, and the effect of acidic FGF on FGFR-3 was well established. In the future, we will discuss in detail the barriers to FGFR-3 expression in different intracellular signaling mechanisms, and predict the effects of FGFR-3 expression on chondrocyte proliferation in cultured chondrocytes.
