アトピー性皮膚炎(AD)発症の基盤となるatopic skinにおけるバリアー機能障害は、角質細胞間脂質とくにセラミドの減少が関与していることが明らかになっている。このセラミド量の減少は今だADと診断し得ない乳児湿疹患者において、家族にアトピー素因を有する群では家族歴のない群に比し、より顕著であることから、遺伝的素因に基づくことが示唆される。セラミド量減少の成因は、角層を用いた検討でスフィンゴミエリン水解活性が著しく亢進し、アシラーゼ活性をもつ別の酵素が誘導されるためと考えられている。そこで、このスフィンゴミエリン代謝異常が遺伝的に規定された異常、すなわち先天的な代謝異常であるか否かを、リンパ球を用いて検討した。AD患者9例(平均17.4歳)の前腕屈側の皮疹部よりテープストリッピングにて採取した角層と、同一患者の末梢血リンパ球を材料とし、アシラーゼ活性を測定した。その結果、角層のアシラーゼ活性は正常人に比し著しく亢進していたが、リンパ球においては有意な変動は認められなかった。本法は、侵襲も少なく乳児患者での検討も可能と思われ、今後症例数を増やすとともに乳児湿疹患者あるいはアトピー素因を有する乳児においても検討する必要があると思われる。
