【対象と方法】1993年から1995年までの胃癌46例と大腸癌46例を対象とし、原発巣から採取した癌組織をGTCにて溶解し、超遠心法によりtotal RNAを取り出して特異的Primerを用いたRT-PCRを行なった。転移因子として注目されているMMP-9とその活性化因子であるuPA、さらにMMP-9の抑制因子であるTIMPの3種を標的にしてm-RNAの発現を検索した。これらの発現と臨床病理学的所見(特に転移形式)や予後との関連を検討した。【結果】1)MMP-9は胃癌の46例中25例(54.3%)と大腸癌で46例中26例(56.5%)に、uPAは胃癌の35例中22例(62.9%)と大腸癌で40例中31例(77.5%)に、TIMPは胃癌の43例中34例(79.1%)と大腸癌で37例中31例(83.8%)に認められた。MMP-9とuPAは同時発現例が多かった。2)MMP-9については各病理学的因子との関連を検討すると、胃癌において総合的進行程度が高いほどMMP-9の発現率が有意に高く(p<0.05)、転移との関連ではリンパ節転移の有無と相関があり(p<0.001)、リンパ節転移程度が高くなるにつれてMMP-9の発現率が有意に高くなった(p<0.001)。またMMP-9陽性群は腹膜播種が多く認められた(p<0.01)。脈管侵襲ではly因子の程度と相関した(p<0.001)。さらにMMP-9陽性群は有意に予後が不良であった(p<0.05)。大腸癌ではMMP-9陽性群に肝転移が多く認められ(p<0.05)、また腹膜播種が多い傾向にあった(p<0.1)。しかし脈管侵襲では関連を認めなかった。3)uPAについては胃癌における検討で総合的進行程度と相関した(p<0.001)。uPA陽性群はリンパ節転移の有無およびリンパ節転移程度と相関し(p<0.01)、また腹膜播種が多く認められた(p<0.05)。さらにuPA陽性群は壁深達度が高かった(p<0.05)。脈管侵襲ではv因子陽性例が多い傾向があった(p<0.1)。大腸癌についてはuPAと関連のある因子は認められなかった。4)TIMPについては胃癌と大腸癌のいずれも関連のある因子は認められなかった。今後は以上の結果をふまえてその他の因子に関しても有機的に検討していくつもりである。
