ラット軟骨肉腫に対してElectrochemotherapy(以下ECT)を行い、その効果と有効な抗癌剤及びその濃度、副作用について調べた。治療は、薬剤を尾静脈から静注後、腫瘍の両端の皮下に刺入した電極間に高電圧負荷をかけるもので、電圧は700V/CM、持続時間は100μSEC、回数は1秒間隔で計8回行った。まず抗癌剤として、ブレオマイシン、シスプラチン、メソトレキセート、アドリアマイシン、サイクロフォスファマイドの5種類について濃度を10%LD50に統一し、腫瘍発育抑制実験を行ったところ、ブレオマイシンと高電圧負荷との併用のみが顕著な効果を示した。次に高電圧負荷の有無の2群で、治療直後、及び30分後に採取した腫瘍組織内のブレオマイシン濃度は治療直後、30分後のいずれにおいても、高電圧負荷をかけた群において、有意に高かった。ブレオマイシンの濃度は1%、5%、10%LD50としても同様に有効であり、その最小有効濃度は1%以下である可能性が示唆された。また腫瘍の大きさの異なる2群に対する腫瘍発育抑制効果に明らかな差は認められなかった。本療法が有効な理由は、高分子であるため腫瘍細胞内への取り込みが非効率的なブイオマイシンが、高電圧負荷によるelectropore形成により効率的に取り込まれたためと考えられ、臨床的には、重篤な副作用が生じていないことからも制癌剤、放射線などの補助療法が無効なヒト軟骨肉腫にも有用となるうる。また全切除困難な腫瘍の治療や補助療法の一つとしての可能性もある。ただヒト軟骨肉腫のような大きな腫瘍に有効か否かが臨床応用上の問題と思われる。以上の結果は、第28回日本整形外科学会骨軟部腫瘍学術集会にて発表した。
