硫酸マグネシウム(MgSO_4)による低血圧麻酔の可能性について検討した。【対象および方法】肝腎障害および神経・筋疾患のない予定手術患者(ASAI-II)7名を対象とし、麻酔は亜酸化窒素(4L・min^<-1>)・酸素(2L・min^<-1>)・セボフルラン2-3%にて維持した。低血圧麻酔は、執刀15分後の時点で、硫酸マグネシウム30mg・kg^<-1>をbolus投与した後に、2g・h^<-1>の投与速度にて持続投与することにより導入した。測定項目は、収縮期血圧(SAP)、拡張期血圧(DAP)、平均血圧(MAP)、心拍数(HR)、血中ストレスホルモン(エピネフリン、ノルエピネフリン、ドパミン、アルドステロン、ADH、レニン活性)、血中マグネシウム、Na^+、K^+およびCa^<2+>濃度とした。測定時期は、麻酔導入前、MgSO_4投与前、投与後60分、120分および投与終了後60分の各時点とし、導入前値と比較した。【結果】MAPは、導入前の95±3mmHgから、MgSO_4投与後60分および120分の各時点において、それぞれ67±2および65±2mmHgへと有意に低下した。HRは、88±10beats・min^<-1>から、MgSO_4の投与後60分および120分の各時点において、それぞれ82±7および79±6beats・min^<-1>へと減少する傾向を示した。MgSO_4投与による心電図変化は認められなかった。投与終了後60分までのいずれの時点においても反跳性の血圧上昇は認められなかった。各ストレスホルモン、血中Na^+、K^+およびCa^<2+>濃度には有意の変動は認められなかった。血中総Mg濃度は、4.4-5.1mg・dL^<-1>にて推移した。【結論】亜酸化窒素・酸素・セボフルラン麻酔下におけるMgSO_4持続投与に伴う動脈血圧、心拍数、心電図および血中ストレスホルモン濃度の変動から、MgSO_4による低血圧麻酔の臨床応用が可能であることが判明した。低血圧による反射性の頻脈および投与中止後の反跳性の血圧上昇が認められないことがMgSO_4による低血圧麻酔の利点だと考えられる。(第18回日本麻酔・薬理学会総会に発表予定,1996年6月13,14日)
