抗リン脂質抗体、特に抗カルジオリピン抗体との結合に、ある種の血清蛋白(抗カルジオリピン抗体コファクター: ex. β2-GlycoproteinI、以下β2GPI)の存在が必須である。最近開発された抗カルジオリピンキット「ヤマサ」EIAを用い、反復初期流産症例・妊娠中毒症を伴わない子宮内胎児発育遅延(以下IUGR)症例、pregnancy induced hypertention(以下PIH)症例および妊娠12週以降の中期流産を含む子宮内胎児死亡(以下IUFD)症例の各疾患におけるβ2GPI依存性抗カルジオリピン抗体を測定し検討を行った。【対象】当院習慣流産外来を受診した連続2回以上の初期流産を有する症例72例、当院で正期産した妊娠中毒症を伴わないIUGR症例13例、当院で分娩したPIH症例22例、および妊娠12週以降の中期流産を含むIUFDまたはIUFDの既往を有する症例30例を対象とした。【結果】正常対照群は正常妊婦175例で、その99パーセンタイル値である1.3unit/mlを正常カットオフ値とし、それ以上を陽性とした。統計的有意差はウイルコクソン検定(有意水準5%)を用いた。反復流産症例における陽性例は2例(2.8%)で、正常対照群との間に有意差は認めなかった。そのうち12.2unit/mlと高値を示したのは、SLE症例であった。IUGR, PIH症例は、両症例とも全例陰性であった。IUFD症例では、30例中3例(10%)が陽性で、統計上有意差を認めたが、3.4unit/ml、125unit/ml以上を示した2例は、いずれもSLE症例であり、これら膠原病の症例を除外して検討すると有意差は認めなかった。【結論】SLEなどの膠原病患者においては、β2GPI依存性抗カルジオリピン抗体が関与する可能性が推察されるが、膠原病を伴わない反復初期流産・IUGR・PIH・IUFD症例でのβ2GPI依存性抗カルジオリピン抗体の関与は確認できなかった。
