教室の膵切除例の臨床的検討により,術後膵性糖尿病を来す要因には,Type1:膵実質量減少型,Type2:膵管閉塞型,Type3:残膵血流減少型,Type4:膵β細胞疲弊型,Type5:原因不明の5つのTypeに分けられることを明らかにしてきた。従来Type1とType2に関しては様々な観点から検討されてきた。今回我々は新しくType3,4のパターンがあることを認識し,これらの発現原因を明らかにする目的で今回の研究に入っている。従来術後膵性糖尿病がいつ発現したか正確に捉えることは極めて困難であった。これは経時的な膵β細胞機能の変動指標をO-GTTかIV-GTTのΣIRIにておこなっていたためであり,この検査では変動の激しい膵β細胞機能を追跡調査することが不可能である。そこでわれわれは早朝1回の採血にて検査が可能な空腹時CPR変動率という新しい指標を求め,これによりβ細胞機能が回復しているのか低下しているのか随時判定可能となった。さらに空復時CPR変動率が低下した症例では直ちにMRP(MR-pancreatography)を施行し,残存膵管の径を測定,さらにカラードップラー血流計にて背側膵動脈または膵尾動脈血流を測定してきた。これによれば術後3カ月までは全例空腹時CRR変動率の低下を認め,膵β細胞機能は低下している。しかしほとんどの症例はこの段階から回復過程に入り,術後l年目にはほぼ正常領域に復帰する.しかし3力月日以降も低下する症例では,膵管径は正常であるにもかかわらず残膵血流量が抵下していることが明らかとなった。また長期にわたり空腹時CPR変動率が低下してきた症例では,胃潰瘍,肝膿瘍,急性膵炎などを合併した後,急速な残膵血流量の低下を認めた。今後はこれらの原因によらないType5の解明を求めるべく,膵切除時に採取してある膵組織から抗64kD抗体,DQ遺伝子,ICA(Islet cell antibody)を検索し,再手術施行例での再採取膵組織と比較検討し,分子生物学的な観点から発生原因を探索する予定である。

The main causes of diabetes mellitus after surgery are: Type1: reduced body mass,Type2: reduced blood flow,Type3: reduced residual blood flow,Type4: beta cell fatigue,Type5: unknown cause. Type1 and Type2 are related to each other. This paper presents a new Type of research on Type3,4 and its causes. Diabetes mellitus after surgery is difficult to detect. It is impossible to trace the change of β-cell function in the study of O-GTT, IV-GTT and IRI. In the morning, the first round of blood sampling was examined, and the CPR fluctuation rate on an empty stomach was determined. The beta cell function was recovered, and the new index was determined at any time. In the case of low CPR rate, MRP(MR-pancreatography) was performed to determine the diameter of the remaining coronary artery, and the dorsal coronary artery and caudal coronary artery blood flow were measured. 3 months after operation, CRR activity rate decreased in all cases, and β-cell function decreased. The symptoms were divided into two stages: the recovery process, and the recovery process in the normal field one year after operation. In the case of low blood flow, the diameter of the blood vessel is normal. The CPR rate is low on an empty stomach for a long time, and the symptoms include gastric ulcer, liver pustules, acute inflammation, and rapid residual blood flow. In the future, we will explore the causes of Type5 infection by using anti-64kD antibody,DQ gene and ICA(Islet cell antibody) during resection, and by comparing tissues and molecular biology during reoperation.
