これまで歯周炎の病態解明のために多くの研究者が歯周炎患者の歯肉溝滲出液中に存在するサイトカインなどの炎症性因子の動態を調べてきた。しかし採取可能な歯肉溝滲出液が微量であるため、一歯単位の評価が非常に困難で、多くの報告では患者単位での比較を行うのみであった。一方、歯周炎の進行の程度は個々の歯によって非常にばらつきがあり患者単位の比較では詳細な臨床症状との関連性を見いだすことは不可能に近いと思われる。しかし一歯単位で調べることが可能になれば歯周病における骨吸収ならびに予後の推察、治療に対する反応性をより詳細に解明できると確信し研究を行った。その結果、今回の研究により約70歯の歯肉溝滲出液中のIL-1を調べたところ、一歯ごとに詳細に歯周炎の程度を反映していることが明かとなった。またなかでもIL-1raに関しては病態の程度に依存せず、むしろ病態が重症になるに従い減少している傾向が認められた。このことは歯肉溝滲出液のみに認められる傾向だはなく、ホモジナイズした歯肉組織中でも同様の傾向が認められた。そこで我々はIL-1とIL-1raの割合を比率として求め、既存の臨床パラメーターに如何に相関しているかについても検討を加えたところ相関係数の有意性をもって相関することが明かとなった。現在この結果についてはJournal of Periodontal Researchに投稿中である。さらに我々は歯肉を凍結切片にし免疫染色の手法を用いることによりこれらサイトカインの局在について調べているところである。
