著者らは、長年にわたり、バイオフィードバックを利用した咬合診査法を顎関節症患者に応用し、良好な成績をおさめている。本研究では、咬合調整のみで治癒した患者群を対象とし、個々の症例の顎関節症発症に関与していた咬合異常部位について検索し、これらの咬合接触点数と頭頸部筋群の疼痛発現部位および緊張性頭痛との関連を検討した。被験者は、当科に来院した顎関節症患者で、修復物が少なく完全歯列を有し、通法の咬合診査では咬合高径や咬合位の明かな異常を認めない40症例を用いた。これらの被験者は、バイオフィードバックを用いた咬合診査によって検出された咬合異常部位の咬合調整により完全治癒した患者である。(1)本研究の被験者は、治療前に咬合接触点数の著しい左右的不均衡が認められ、また症状消失時にはこの不均衡が有意に改善していることが明らかになった。(2)歯種別では、初診時での第二大臼歯の咬合接触点数は、他の歯種に比較し有意に多い値を示した。症状消失時では、初診時に比較し、第二小臼歯と第一大臼歯の接触点数は有意に増加し、第二大臼歯は有意に減少した。(3)さらに、症状が片側のみに認められた症例を用いて、症状側と非症状側との咬合接触点数を比較したところ、症状側の咬合接触点数が有意に多いことが判明した。また、症状側の大臼歯の咬合接触点数は、非症状側の大臼歯に比べ有意に多い値を示した。(4)各臨床症状のうち、自発痛、運動痛、咀嚼痛および顎関節部、咬筋深部、顎二腹筋後腹の圧痛は、咬合接触点数の多い側に一致して観察された。(5)これらの結果から、顎関節症患者の頭頸部筋群の疼痛の発現および緊張性頭痛は、咬合異常部位と密接な関連を有し、なかでも咬合接触点数の左右バランスが大きく関与していることが示唆された。

For example, if the patient has been diagnosed with jaw joint disease for many years, he or she will have good results. This study examined the relationship between occlusal adjustment and the onset of jaw joint disease in patients with occlusal adjustment, occlusal contact points, and pain onset in head and neck muscle groups. The patients with jaw arthrosis were admitted to the hospital, and the prostheses were used in 40 cases with complete occlusal alignment and abnormal occlusal position. The patient was completely cured by occlusal examination and occlusal adjustment. (1)In this study, the imbalance of occlusal contact points before and after treatment was recognized and the imbalance was intentionally improved when symptoms disappeared. (2)The number of occlusal contact points of the second largest tooth at the time of initial diagnosis was significantly higher than that of the second tooth. When symptoms disappear, the contact points of the second and the first teeth are intentionally increased, and the contact points of the second teeth are intentionally decreased. (3)The number of occlusal contact points on the symptom side and the non-symptom side are compared. The number of occlusal contact points on the symptomatic side of the tooth is higher than that on the non-symptomatic side. (4)All clinical symptoms, spontaneous pain, exercise pain, chewing pain, jaw joint pain, deep jaw muscles, pressure pain in the posterior jaw muscles, and occlusal contact points were consistent. (5)The results showed that the occurrence of pain in the head and neck muscles of patients with jaw arthrosis was related to the close contact of abnormal occlusal parts and the number of occlusal contact points.
