1996年8月,岩手県盛岡市の1歳6ヵ月児健康診査を受診した214名(男:119名,女:95名)の児の母親を対象として,児の食習慣,日常生活習慣および母親の歯科保健に対する意識と態度に関する25項目を調査し,それらの結果及び1歳6ヵ月健康診査票の記載事項と歯科保健行動の実践との関わりを分析した.母親による仕上げ磨きの有無により対象者を2群に分け,plaque scoreの平均値の差を検定したところ,実行している群のscoreが有無に低い(p<0.05)ことから,仕上げ磨きの有無を健康的な口腔衛生状態を促進する保健行動の1つとして分析の指標とした.その結果,毎日自分で歯を磨く児の母親,虫歯の重大性の認識が高い母親,児とゆったり過ごす時間がある母親ほど仕上げ磨きを欠かさず行う傾向にあった(p<0.01,p<0.01,p<0.05).一方,父親の育児協力の有無によって,母親がゆとりをもって過ごす時間の有無に差があるか否かを分析した結果,父親の育児協力が得られている母親は児とゆったりと過ごす時間があると感じる傾向にあった(p<0.01).さらに,仕上げ磨きの有無を従属変数,調査項目を独立変数とした重回帰分析(stepwise)を行った結果,「虫歯の重大性の認識」,「母親の力による虫歯予防の可能性の認識」,「児の歯の脆弱性の認識」,「母親の健康に対する意識」,「児自身による歯磨き」および「母親が児とゆったりと過ごす時間の有無」の6項目が選択された(R=0.374,p<0.01).以上の結果から,Health Belief Modelの諸要因が歯科保健行動を規定する要因として有効であり,また,父親の協力というsocial supportが保健行動への動機付けの強化に関わっている可能性が示された.今後,対象児が3歳になった時点で再調査し,結果を分析することにより,予防的歯科保健行動を規定する諸要因をより明確な形でモデル化することができると考える.
