项目摘要

(1)文献レビュー:癌患者のためのグループ・アプローチは情報的サポートグループの提供を主な目的とする教育的介入と情緒的サポートの授受に焦点がおかれるGroup Discussion介入に分けられ、心理的適応に及ぼす効果は前者の方が大きいという報告があり、我々のグループ・アプローチの評価に当たって検討すべき点である。(2)質問紙の改良:癌患者のためのサポートグループ研究を行うに当たって、昨年度の質問紙を改良した。対象者を選定するための調査においても心理テストはPOMSに統一し、比較検討しやすいようにした。(3)乳癌患者のためのサポートグループ研究:前年度の反省からフォローのセミナーを取り入れたことによってその期間が、セミナーでの体験をより確かなものにするための大切な時間となることが明らかになった。今年度はセミナー参加後の影響を中心に分析した結果、それらのカテゴリーは、癌等の難治性疾患患者の生活の質を高めるケアを行うために必要な「身体的側面」「心理的側面」「社会的側面」「spiritualな側面」すべての側面に影響していることがわかった。ビデオ録画を行うことによって、表情や仕草から、参加者の発言の意味するところをより多角的に分析できた。また、継続的サポートグループと短期型サポートグループとを比較すると、継続的サポートグループは短期型のような密な連続性はないが、『居場所』的存在になっていることが明らかになった。(4)医療スタッフを対象としたサポートグループ研究:看護婦以外の職種も加えた集中的なサポートグループを行った結果、看護婦だけのグループとは異なる特徴が現れ、グループ・ダイナミックスも深まり、スタッフのための精神的ケアとして意味がある感触を得た。
