项目摘要

Elderly population will increase to quarter of population in Japan. Discharge planning for hospitalized elderly patients is very important in special function hospitals and university hospitals. Department of medical social work and discharge planning started discharge planning in 1997. We have studied the effectiveness of discharge planning for hospitalized patients in university hospital of Tokyo. Discharge planning team is made of doctor, medical social worker and home care coordinator (nurse). Discharge planning team did discharge planning of 302 patients. Discharge planning team coordinated with 68 home care nursing stations. Number of patients who chosen home care instead of long hospitalized care increase up to about twice. Home care coordinator informed goodness and risk of home care. Discharge planning team is effective to shortening hospitalized day.

老年人口将增加到日本人口的四分之一。老年住院患者出院计划是专科医院和高校医院的一项重要工作。医务社会工作及出院策划部于1997年开始推行出院策划。我们研究了东京大学附属医院住院患者出院计划的有效性。出院计划小组由医生、医务社工和家庭护理协调员（护士）组成。出院计划小组对302例患者进行出院计划。出院计划小组与68个家庭护理站协调。选择家庭护理而不是长期住院护理的患者人数增加了约两倍。家庭护理协调员告知家庭护理的好处和风险。出院计划组对缩短住院日有显著效果。
