项目摘要

公衆衛生学関連教育は、医療改革に対応し教育戦略と方法の変革が社会的ニーズとなっている。本研究では、医学生に対し「主体的問題解決型教育」「保健医療福祉の統合教育」「感性とコミュニケーション能力の開発教育」を特徴とした「地域コミュニティ基盤実習教育」を23年間展開し、21世紀にむけた公衆衛生教育理論と技術開発を行った。具体的には、医学生2人が1組となり1年間定期的に大学の存在する出雲市内の家庭(要介護老人家庭、独居老人家庭、障害者家庭、通常家庭など)を訪問し、家庭健康管理実習(Family Health Practice)を行った。さらにコミュニティの健康福祉問題を自主研究として取り組み、コミュニティヘルス調査研究(Community Health Practice)を行った。さらに希望学生に対しては、精神病院体験入院、福祉施設におけるケアマネージメント、市立診療所・福祉施設におけるプライマリケア実習などを選択コースとして行った。このコミュニティ基盤教育の評価を行った結果、地域社会の住民、諸保健医療福祉施設、行政、医師会など社会的資源を充分有効に活用できること、病院では困難な住民の健康と疾病の自然誌を理解することができること、住民や保健医療福祉スタッフとのコミュニケーション能力が磨かれ、将来医師となった時に協働すべき他職種とのパートナーシップを確立することができること、プライマリヘルスケアの重要性や家庭医学と公衆衛生との連携の重要性などを認識することができることなど、今後の公衆衛生教育の有効な教育モデルであることを明らかにした。今後の課題は、大学の独立行政法人化に対応し、個性ある大学教育の一環として地域ニーズに対応した医学教育システムを全国の大学がコミュニティの中に構築することであろう。
