Oxygenation status of head and neck tumor evaluated with near infrared reflection spectroscopy

Using a non-invasive system with near infrared reflection spectroscopy (NIR), we studied the oxygenation status of primary and metastatic lesions of the head and neck tumors. In the fiscal year of 1999, we studied the status of the oxyhemoglobin (oxyHgb) and in the fiscal year of 2000, we analyzed the status of the cytochrome aa3 (Cyt).In the head and neck region, the mucosa were found to be higher oxyHgb values than the skin. Especially, buccal mucosa showed the highest. There were no correlations between oxyHgb and PaO_2 and between oxyHgb and peripheral blood Hgb. Tissue oxyHgb levels measured using NIR were not affected by oxygenation status of the blood. OxyHgb values in all metastaic lymph nodes were significantly low compared with normal tissues suggesting hypoxia. Primary lesions showed either normal or low oxyHgb suggesting normoxic and hypoxic status of the tumor.Cyt levels in head and neck tumors were significantly low compared with normal tissues. There were no correlations between Cyt and PaO_2 and between Cyt and peripheral blood Hgb. Cyt values in the normal tissues and tumors were not affected by the status of blood. Combined with the data obtained from oxyHgb and Cyt, it suggested that anaerobic and aerobic metabolism was the main source for tumors and for the normal tissues, respectively.For the terms of 1999 and 2000, we studied oxygenation status for normal tissue and tumors in humans using NIR.It was important to estimate the tumor hypoxia because of a factor for radioresistance. We conclude NIR is a useful method for radiation therapy.
