本研究は、精神科病院に長期在院をしている統合失調症(精神分裂病)患者を社会復帰へ促すために患者自身の状況と彼らを取り巻く治療的環境を分析し、これらのどの視点、あるいはどの要因からのアプローチが社会復帰に有効であるのかを明らかにするのを目的にした。そのためにまず、長期在院患者の状況を把握するために平成11年には患者の言動の質的分析、翌年には長期在院患者のいる病棟に勤務する看護師(n=27)の気持ちや考え、勤務動機を中心に質的研究を行なった。この2つの予備研究から現在の状況で患者の社会復帰を促進するためのプログラムには、その対象を患者よりも看護師とし、先に働きかけるほうが有効であり、特に看護師の病院組織へのコミットメントと専門職性の関係が重要な鍵になることが明らかになった。そのため、その関係を明確にするために平成13年度の最終調査では、精神科看護師を対象として4種類の尺度-AllenとMeyerの3次元組織コミットメント尺度、ミネソタ職務満足度尺度、看護師の専門職性測定尺度(NPS21項目)、ローゼンバーグの自尊感情尺度-で測定した。データは、日本の13ヶ所の精神科病院に勤務する看護師(准看を含む,n=1313)から得られ、共分散構造分析が行われた。その結果、現在の精神科看護師の特徴として(1)継続的コミットメント(離職に関してリスクを感じる場合)が情動的コミットメント(病院組織への愛着や同一視)よりも高く、(2)情動的コミットメントは、看護師としての専門職性よりも職務満足に影響することが明らかになった。精神科看護師の患者観と専門的行動は多様であり、それは組織のシステムの特徴、たとえば、職場での人間関係や病院の掲げる目標や価値に影響される。そのため病院組織の改善により患者ケアの質の向上が期待でき、患者を社会復帰を目的としたケアの効率が上がることが示唆された。
