作業療法士には対象児の活動を十分に観察,解釈した上で,介入する力が求められる。作業療法過程における活動の詳細な解明こそ,介入の質の向上を計るために必要なことである。学生に対しては、発達や障害に関する科学的知識を,児への多様な活動へと適応させていくための方法論提示が重要となる。作業療法士の役割は児の障害を軽減し発達を促すことだが,それを説明するのは,現場の事実に即した。発達理論の構成である。本研究では作業療法場面をとりあげ,その過程の検討を試みた。記述にはサーサスの方法を修正した以下に1例を示す。運動障害児,作業療法士,母親が参加した。母親は児の遊びの広がりを求め,作業療法士は,対人関係を心地よいものと児が感じる中で遊びの幅を広げようとした。活動は8つに分類できた。総記述数849行であった。1)1〜107:児と追いかけっこ,2)108〜245:一緒におもちゃで遊ぶ,3)246〜373:スーパーボールを放る,4)374〜560:キャッチボール,5)561〜671:手中のボール当てゲーム,6)672〜772:スイッチ遊び,7)772〜849:終了後の交流であった。次に詳細に検討した結果,作業療法士は児を理解するために児の行為や発声を解釈しながら作業療法を進めていた。過程においては,児をガイドし,活動を持続させ,複雑化させていった。1例では,遊びの文脈をつくり,児の積極参加を導き,呼び声をひきだした。これは,児の活動に対するシェマを一歩発展させたものであった。作業療法士は児との相互作用を通して,活動への能動的参加を導き,言語,思考,感情などの機能を引き出した。作業療法士は刻々と活動に反映した発達の最近接領域を評価し,活動の深化を導いた。これより,本研究成果は,学生には作業療法現場の実態の説明となり,臨床家には,自らの作業療法を振り返るためのリソースの提示となると考えられた。
