本研究の対象は、宮城県14町村に住む40-64歳から構成されるコホート研究の女性24,769人である。同コホートは平成2年にベースライン調査が実施され、その後追跡調査が継続されている。これらの対象者について、宮城県新生物レジストリー委員会の承認を得て宮城県地域がん登録資料との照合を実施し、平成9年末までの乳がん罹患状況について調査した(罹患数141例)。その上で大豆製品、血漿ゲニスタイン推定値と乳がん罹患との関係を、Coxの比例ハザードモデルを用いて交絡要因を補正したハザード比を求めることにより検討した。その結果、大豆類については、週1〜2回以下摂取群を基準とした時の毎日摂取のハザード比は1.08(0.64-1.80)であり、摂取頻度の増加によるリスクの低下を認めなかった(trend p=0.774)。また味噌汁に関しては、一日1杯以下摂取群を基準とした時、一日3杯以上摂取群のハザード比は0.81(0.51-1.29)と統計的に有意なリスク低下ではなく、また傾向性検定においても有意差を認めなかった(trend p=0.381)。また血漿ゲニスタイン推定値との関連については、四分位で最も推定値が低い群を基準とした時、他の3群のリスクは低値群から高値群に向かって0.86(0.54-1.37)、0.90(0.55-1.47)、0.66(0.39-1.14)であり、傾向性は有意ではなかった(trend p=0.175)。一方、30歳時味噌汁摂取杯数と乳がん罹患リスクとの関連については、一日摂取杯数が1杯以下を基準とした時、3杯、4杯以上のハザード比はそれぞれ0.64(0.36-1.12)、0.47(0.22-1.04)であり、一日3杯以上でリスクの低下を認めた。この30歳時の味噌汁摂取と乳がんリスクとの関連は有意な傾向性を示した(trend p=0.006)。
