敦賀湾における核事故を想定した医療対応力に関して、北陸・東海・近畿の部の医療機関にアンケート調査を実施した。その結果、ヨウ素製剤の備蓄・重症熱傷や骨髄抑制の治療などの急性期治療は、限定された人数なら対応可能であるが、大規模事故にて多数の被爆者が出た場合は対応が困難であることが推定された。敦賀湾限定の核事故におけるヨウ素製剤投与に関しては、確認された備蓄量11万人分という数量から推測して、準広域にて十分な対応が可能と思われるが、大都市を含む大規模災害となった場合の必要数と供給には2桁ほどの乖離が予想された。広域避難に対しては転送手段・受け入れネットワークにも課題が確認された。また、NBC災害や大規模災害に対する災害対応マニュアルを含めた準備態勢にも問題が見受けられた。一般施設の被災に関して、施設間転送ネットワークや各種災害マニュアルなどは比較的低予算で整備することが出来、ほかの各種災害に援用可能なシステムもあるので、積極的な整備が望まれる。災害拠点病院や県立病院単位でのネットワークは整備されているが、ネットワーク外に置かれている私立病院をはじめとする施設と患者が存在する。特に広域避難に関しては、個々の施設や自治体の対応の限界を超えた問題が多い。行政や関連学会の補助が必要と考えられるなど、今後の対策要件等について明らかにすることができた。避難区域が広域となった場合や大都市が発災中心となった場合、さまざまな医療措置が不足となる事態が想定される。例えば本研究の調査では、人工透析通院数と余剰受け入れ可能数の乖離が確認された。政策における余剰医療設備の適正量の決定は、医療経済的な側面からだけではなく社会安全保障の側面からの検討も必要との示唆を得た。
