Role of gastric leptin and cholecystokinin in patients with functional dyspepsia

研究对象为功能性消化不良患者和健康志愿者。患者均给予书面知情同意，接受内窥镜下的血液取样和胃粘膜活检。本研究经我院伦理委员会批准。使用RIA试剂盒检测血浆CCK和瘦素。在内窥镜检查时对基底部位进行活检。瘦素免疫染色采用兔抗人瘦素抗体（a -20, Santa Cruz Biotechnology, California， USA）。血浆CCK和瘦素水平在功能性消化不良患者和健康志愿者之间没有显著差异。在军团上皮腺的下半部分发现瘦素免疫反应细胞。上皮细胞中瘦素阳性染色强度评分为5分。功能性消化不良患者与健康志愿者瘦素阳性染色强度无显著差异。此外，瘦素阳性染色强度在幽门螺杆菌阴性组与阳性组、高（≧23）与低（<23）体重指数组、老年（≧70）与非老年（<70）体重指数组之间无显著差异。胃黏膜瘦素mRNA和十二指肠黏膜CCK mRNA的含量有待进一步测定。

The patients with functional dyspepsia and healthy volunteers were investigated in the study. They all gave written informed consent for blood sampling and gastric mucosal biopsy by endoscopy. This study was approved by the Ethics Committee of our institute. Plasma CCK and leptin were estimated using a RIA kit. Biopsies of fundic sites were performed during endoscopic examination. Immunostaining for leptin was performed by a polyclonal rabbit antihuman leptin antibody (A-20, Santa Cruz Biotechnology, California, USA). Neither plasma CCK nor leptin levels differed significantly between the patients with functional dyspepsia and healthy volunteers. Leptin immunoreactive cells were found in the lower half of the epithelial glands of the corps. Intensity of positive staining for leptin in the epithelial cells was rated on a 5-point score. There was no significant difference in intensity of positive staining for leptin between the patients with functional dyspepsia and healthy volunteers. In addition, there was no significant difference in intensity of positive staining for leptin between the H. pylori-negative and -positive groups, between the high (≧23) and low (<23) levels of body mass index groups or between the aged (≧70) and not aged (<70) groups. Further determination of gastric mucosal leptin mRNA and duodenal mucosal CCK mRNA was required.
